Anjiotensin dönüştürücü enzim inhibitörü enalapril ile anjiotensin II reseptör antagonisti losartanın hafif ve orta derecede hipertansiyonu olan hastalarda antihipertansif etkinliklerinin karşılaştırılması

Özet
AMAÇ: Anjiotensin dönüştürücü enzim inhibitörü (ADEİ) ve Anjiotensin II (AT II) reseptör blokeri antihipertansif ilaçlar hipertansiyon tedavisinde yaygın olarak kullanılmaktadır. Bu iki ilacın antihipertansif etkinliğini karşılaştırmayı amaçladık. GEREÇ VE YÖNTEM: Bir ADEİ olan enalapril ile AT II reseptör antagonisti losartan hafif ve orta dereceli hipertansiyonda, antihipertansif etkinlik açısından 8 haftalık randomize, çift kör bir çalışma ile önceden tedavi almamış 66 hastada karşılaştırıldı. Birinci gruba (32) enalapril 10mg/gün, 2. gruba (34) losartan 50mg/gün verildi. BULGULAR: Kan basıçları tedavi öncesi, tedavinin 1., 2., 4. ve 8. haftalarında ölçüldü. Serum kreatinin, sodyum, potasyum düzeyleri aylık olarak takip edildi. İki grupta da tedaviyi kesmeyi gerektirecek bir yan etkiye rastlanmadı. Enalapril alan 3 (%9.2) hastada hafif şiddette kuru öksürük oluştu. Enalapril grubunda sistolik kan basıncı 8 haftanın sonunda 155.1±4.8 (145-168) mmHg'dan 144.5±4.4 (140-155) mmHg'ya düştü. En anlamlı düşüşler 1. ve 2. haftalarda oldu (p=0.001, p=0.012). Diastolik kan basıncı 93.9±3.3 (89-102) mmHg'dan 8 haftada 87.1±2.7 (80-90) mmHg'ya düştü. Birinci, 2. ve dördüncü haftalardaki azalmalar istatistiksel olarak anlamlı idi (p<0.001, p=0.001, p=0.002). Losartan grubunda kan basıncı sistolik 155.7±3.8 (145-168) mmHg'dan 8. haftada 146±3.5 (140-150) mmHg'ya düştü. En anlamlı düşüş 1. haftada idi (p=0.001). Diastolik kan basıncı 1. haftanın sonunda 93.7±2.6 (88-102 mmHg'dan 91.2±2.8 (88-98) mmHg'ya düştü (p<0.001). Sekiz haftada enalapril grubunda sistolik kan basıncı %7.2±3.3 (2-15), losartan grubunda %6.2±2.7 (0-12) düştü. İstatistiksel anlamlı farklılık yoktu (p=0.272). Diyastolik kan basıncı enalapril grubunda %6.6±4.3 (0-20), losartan grubunda %5.9±3 (0-12) azaldı. Yine istatistiksel anlamlı farklılık yoktu (p=0.782). SONUÇ: Enalapril ve losartanın hafif ve orta dereceli hipertansiyonun tedavisinde etkin ve güvenilir olarak kullanılabileceği sonucuna varıldı.
Abstract
OBJECTIVE: Angiotensin converting enzyme inhibitors and Angiotensin II receptor blockers are used widely as antihypertensive drugs in hypertensive therapy. Enalapril, angiotensin converting enzyme inhibitor, was compared with losartan, angiotensin II reseptor antagonist, about antihypertensive effect in mild and moderate hypertension in this study which was 8 weekly, randomly, double blind and had 66 patients, who had not taken any treatment before then study. MATERIAL AND METHODS: Enalapril 10 mg/day was given to the first group and losartan 50 mg/day was given to the second group. Blood pressures were measured in before and 1st, 2nd, 4th, 8th- weeks of treatment. Serum creatinine, Na and K levels were taken as monthly. Any side effect for interrupting of study was not encounted in the groups. A mild degree cough was seen in 3 patients (9.2 %) taken enalapril. RESULTS: Systolic blood pressure fell from 155.1±4.8 (145-168) mmHg to 144.5±4.4 (140-155) mmHg in the group of enalapril 8 week later. The most expressive decreases were in the 1sth and 2nd weeks (p=0.001, p=0.001). Diastolic blood pressure fell from 93.9±3.3 (89-102) mmHg to 87.1±2.7 (80-90) mmHg for 8 week. Decreases in the 1sth, 2nd, 4th, weeks were statistically meaningful (p=0.001, p=0.001, p=0.002). Systolic blood pressure fell from 155.7±3.8 (145-166) mmHg to 146±3.5 (140-150) mmHg in the group of losartan, 8 week later. The most expressive decrease was in 1sth week (p=0.001). Diastolic blood pressure fell from 93.7±2.6 (88-102) mmHg to 91.2±2.8 (88-98) mmHg at the end of the 1sth week (p=0.001). Systolic blood pressure was found in the group of enalapril fall 7.2±3.3 (2-15) % and 6.2±2.7 (0-12) % fall systolic blood pressure in the group of losartan was found for 8 week. No significant statistically difference was found (p=0.272). Diastolic blood pressure decreased 6.6±4.3 (0-20) % in the group of en alapril and 5.9±3 (0-12) % in the group of losartan. No significant statistically difference was found (p=0.782). CONCLUSION: We thought that enalapril and losartan were effective and safe choices of treatments of mild and moderate hypertension.
