Özet

Amaç: Bu çalışmanın amacı izofluran, sevofluran ve desfluranın hipotermik kardiyopulmoner bypass esnasında juguler venöz bulb oksijen saturasyonu (SjO2) üzerine etkilerinin incelenmesidir. Gereç ve Yöntem: Etik kurul onayı alındıktan sonra, çalışmaya hipotermik koroner bypass cerrahisi geçirecek olan 36 hasta kapalı zarf yöntemi ile üç gruba randomize edildi. Bütün hastalara 0.1 mg kg-1 morfin ile premedikasyon uygulandı. İndüksiyonda ise 1 mg kg-1 propofol, 1.5 mcg kg-1 remifentanil ve 0.1 mg kg-1 panküronyum uygulandı. Anestezi idamesinde 0.4 mcg kg-1dak-1 remifentanil ve Grup İ'ye %0.5-1 izofluran, Grup D'ye %3-3.5 desfluran ve Grup S'ye %1-1.5 Sevofluran uygulandı. Sağ juguler bulb kateter aracılığıyla indüksiyon öncesi (T1), İndüksiyon sonrası (T2), 30.dakika (T3), KPB sonrası 30.dakika (T4), KPB sonrası birinci saat (T5), KPB sonrası altıncı saat (T6), KPB sonrası 12.saat (T7) dönemlerinde SjO2 çalışıldı ve kalp hızı, ortalama kan basıncı not edildi. İstatistiksel analiz amacıyla tek yönlü ANOVA ve tekrarlayan ölçümlerde ANOVA kullanıldı. P<0.05 anlamlı olarak kabul edildi. Bulgular: Gruplar demografik veriler açısından benzerdi. Gruplar arası SjO2 incelemelerinde fark bulunamaz iken, grup içi incelemelerde tüm gruplarda benzer olarak indüksiyon sonrasında artma (p<0.01) ve T4, T5, T6 ölçüm aralıklarında belirgin düşme tespit edilmiştir (p<0.01). Sonuç: Kardiyopulmoner bypass esnasında SjO2'de gözlenen değişiklikler üzerine Sevofluran, Desfluran ve İzofluranın etkileri benzerdir.

Abstract

Objective: The aim of the study was to investigate the effects of isoflurane, sevoflurane or desflurane on jugular venous bulb oxygen saturation (SjO2) in hypothermic cardiopulmonary bypass surgery. Material and Methods: After obtaining the ethical committee approval, 36 patients undergoing hypothermic cardiopulmonary bypass surgery were randomly allocated into three groups with closed envelope technique. Morphine 0.1 mg kg-1 was used for premedication. Anesthesia was induced with IV propofol 1 mg kg-1, remifentanyl 1.5 mcg kg-1 and pancuronium 0.1 mg kg-1. Anesthesia was maintained with 0.4 mcg kg-1min-1 remifentanyl and 0.5-1% isoflurane in Group I, 3-3.5% desflurane in Group D and 1-1.5% sevoflurane in Group S. SjO2 were measured and hemodynamic variables were noted before induction (T1), after induction (T2), 30 min. after induction (T3), 30th min. of cardiopulmonary bypass (T4), 60th min. of cardiopulmonary bypass (T5), 6 hours after cardiopulmonary bypass (T6) and 12 hours after cardiopulmonary bypass (T7). One way ANOVA and repeated measures for ANOVA were used for statistical analysis. P value <0.05 was considered statistically significant. Results: The groups were similar according to demographic data. There was no difference in SjO2 measurements between groups but significant differences were found in each group at T2, T4, T5, and T6 measurements compared to basal measurements (p<0.01). Conclusion: The effects of isoflurane, sevoflurane and desflurane on SjO2, in patients undergoing hypothermic cardiopulmonary bypass surgery were similar.
