26.gebelik haftasında massif proteinüri ile müracaat eden ağır preeklampsi olgusunda konservatif yaklaşımın sonuçlarını değerlendirdik. 26. gebelik haftasında, ellerde şişlik, tansiyon yükselmesi şikayetleri ile kliniğimize ilk kez müracat eden 22 yaşındaki primigravid olgu kan basıncı ölçümünün 160/110 mmHg saptanması üzerine gebelik 1, parite 0, ve preeklampsi ön tanısıyla yatırıldı. Hastada diğer ağır preeklampsi kriterleri mevcut değildi. Fetusun henüz immatür olması, hastada hipertansiyon ve massif proteinüri (24 saatlik idrarda protein miktarı 23481 mg/dL) dışında kliniğinde ve laboratuar tetkiklerinde ağır preeklampsi kriterinin bulunmaması nedeniyle acil doğum düşünülmedi ve konservatif tedavi planlandı. İki hafta boyunca masif proteinüri dışında stabil seyreden olgunun ikinci hafta sonunda doppler akım değerleri incelendiğinde orta serebral arterde redistrübüsyon (PI; 1.00, RI; 0.63), uterin arterde bilateral notch, umbilikal arterde ters akım izlenmesi üzerine fetusun prognozu açısından 12 saat arayla 2 kez betametazon toplam 12 mg IV uygulandıktan sonra hasta sezeryana alınarak canlı, 640 gm, kız bebek doğurtuldu. Bebek maalesef postpartum 3. gününde ex oldu. Gebelik yaşı ne olursa olsun ağır preeklampsi olgularında tek başına proteinüri doğumun acil olarak sağlanması için bir kriter oluşturmamaktadır.

Abstract

We aimed to detect the results of the conservative management of a patient with preeclampsia with massive urinary protein excretion. A 22-year-old pregnant admitted with hypertension and edema in hands and hospitalized with the diagnose of gravida 1, para 0 complicated with preeclampsia, at 26th week of gestation. The patient had no other manifestations of severe preeclampsia. As the foetus was immature and the mother had no manifestations of severe preeclampsia except massive proteinuria (23481 mg/dL/24 h) and hypertension, we managed conservatively. There was no change on the clinical findings of the patient next two weeks instead of existance of massive proteinuria. At the end of two weeks doppler scanning showed of redistrubition in middle serebral arteries (PI; 1.00, RI; 0.63), bilateral notch in uterin arteries and reverse flow in umbilical arteries. Subsequently we delivered the female baby who was weighing 640 gm by cesarean section following betamethasone 12 mg IV injection in twelve hours intervals to the mother. However, the baby died on the third day. Massive proteinuria is not an obligatory factor for immediate delivery by itself in severe preeclampsia in any time at pregnancy.
