Maternal serum alfafetoprotein ölçümü ile nöral tüp defekti riskinin değerlendirilmesi

Özet

Amaç: Toplumumuza prenatal tarama testlerinin önemini kavratmak, bunları yaygınlaştırmak ve test sonuçlarına göre aile ile işbirliği yaparak gerekli önlemlerin alınmasını sağlamak. Gereç ve Yöntem: SSK Ankara Doğumevi ve Kadın Hastalıkları Eğitim Hastanesine 1 Mart 1996-31 Mart 2001 tarihleri arasında, 14-22. gebelik haftasında test yaptırmak üzere başvuran 7918 gebede maternal serumda alfafetoprotein değerlerine bakılmıştır. Alfafetoproteini 2.5 MoM üzerinde bulunan gebelerin testleri tekrarlanmış, tekrarlanan test sonucunda alfafetoprotein değeri 2.5 MoM ve üzerinde olan, ultrasonografinin tanımlayıcı olmadığı gebelere amniosentez yapılarak amnios mayide alfafetoprotein ve asetilkolinesteraz bakılmıştır. Bulgular: Tespit edilen 32 nöral tüp defektli gebeliğin 18'inde alfafetoprotein değeri 2.5 MoM ve üzerinde (dört anensefali, üç spina bifida, üç meningosel, iki meningomyelosel, iki ventrikülomegali, iki spina bifida+ventrikülomegali, bir ensefalosel+hidrosefali, bir myelosel) bulunurken, 14'ünde alfafetoprotein değeri 2.5 MoM altında (10 hidrosefali, iki spina bifida+ventrikülomegali, bir spina bifida, bir ventrikülomegali) bulunmuştur. Testin sensitivitesi %56.25, spesifisitesi %98.6, (+) prediktivitesi %13.84, (-) prediktivitesi %99.82, yalancı (+)'lik oranı %1.4 saptanmıştır. Sonuç: İlk olarak 1996 yılında başlattığımız bu çalışmada, dokuz ayda 488 gebe tarama testi yaptırmak amacıyla müracaat ederken, 2000 yılında aynı sürede bu sayı 1870'e çıkmıştır. Bu veriler aile ve toplum sağlığı konusunda halkımızın duyarlılığını göstermek açısından dikkat çekicidir.

Abstract

Objective: To inform the population about the importance of prenatal screening tests, to spread these tests and to evaluate the anomolous pregnants in the early periods in order to take care with the families. Material and Method: There were 7918 pregnant in the interval of 14-22 gestational week in SSK Ankara Obstetrics and Gynecology Hospital between 1/3/1996-31/3/2001. Maternal serum alpha-fetoprotein levels were evaluated. The tests which were over than 2.5 MoM repeated. In the repeated values if the alpha-fetoprotein levels were still 2.5 MoM or higher and the ultrasonographic results were not diagnostic, the amniocentesis were done. Alpha-fetoprotein and acetylcolinesterase levels were analyzed in the amniotic fluid. Results: Neural tube defects were detected in thirtytwo cases. In 18 of these cases the alpha-fetoprotein levels were equal or over than 2.5 MoM (four cases of anencephaly, three cases of spina bifida, three cases of meningocele, two cases of meningomyelocele, two cases of ventriculomegaly, two cases of spina bifida+ventriculomegaly, a case of encephalocele+hydrocephaly, a case of myelocele). In 14 of these cases the alpha-fetoprotein levels were less than 2.5 MoM (10 cases of hydrocephaly, two cases of spina bifida+ventriculomegaly, a case of spina bifida, a case of ventriculomegaly). The parameters found in this study were as follows: sensitivity 56.25%, specificity 98.6%, positive predictive value 13.84%, negative predictive value 99.82%, false positive rate 1.4%. Conclusion: In the year 1996, the first year of this study, in 9 months 488 pregnants applied to have a screening test. In the year 2000, in the same period 1870 pregnants applied to have a screening test. These results are important about the sensitivity of population in the family and society health.
