Tip II diabetes mellituslu hastalarda plasma solubl intersellüler adezyon molekülü-1 düzeyleri ve retinopati ile ilişkisi

Özet

Amaç: Tip-2 diabetes mellituslu hastalarda mikrovasküler komplikasyonların gelişiminde adezyon moleküllerinin olası rollerini araştırmak amaçlanmıştır. Gereç ve Yöntem: SSK Ankara Eğitim Hastanesi 3. Dahiliye polikliniğine başvuran tip 2 dibetes mellitusu olan 50 hasta çalışmaya dahil edildi. Hastalar göz dibi muayenelerine göre diabetik retinopatisi olmayan, nonproliferatif retinopatisi ve proliferatif retinopatisi olanlar şeklinde gruplandırıldı. Kontrol grubu herhangi bir hastalığı olmayan 25 kişiden oluştu. Tüm hastaların açlık ve postprandial plazma glukoz düzeyleri, glikozile hemoglobin (HbA1C) düzeyleri ve solubl intersellüler adezyon molekülü-1 (sICAM-1) düzeyleri ölçüldü. Bulgular: Diabetiklerde sICAM-1 düzeyleri, kontrol grubuyla karşılaştırıldığında istatistiksel olarak anlamlı derecede yüksekti (diabetik hastalarda 500.986±134.35 ng/ml, kontrol grubunda 404.316±104.90ng/ml, p<0.01). sICAM-1 düzeyleri retinopatisi olan grupta retinopatisi olmayan gruptan daha yüksekti (retinopatisi olan grupta 565.17±117.20 ng/ml, retinopatisi olmayan grupta 431.45±117.56 ng/ml, p<0.01). Proliferatif retinopatisi olan grupta ise nonproliferatif retinopatisi olan grupla karşılaştırıldığında sICAM-1 düzeyi istatistiksel olarak anlamlı derecede yüksek olarak bulundu (624.51±114.23 ng/ml'e 495.94±78.02 ng/ml, p<0.05) Sonuç: Diabetes mellitusun mikrovasküler komplikasyonlarında adezyon moleküllerinin rolleri olması muhtemeldir. Bu durumu daha iyi açıklayacak geniş hasta popülasyonunda yapılacak prospektif takip çalışmalarına ihtiyaç vardır.

Abstract

Objective: The aim of this study is to investigate the possible roles of adhesion molecules in pathogenesis of microvascular complications of diabetes mellitus. Material and Method: 50 type-2 diabetic patients who applied to Ankara Social Security Hospital internal medicine outdoor clinic were involved in the study. The patients were groupped according to their retinal examinations as the patients without retinopathy, patients with nonproliferative retinopaty and with proliferative retinopathy. Control group was consisted of 25 healthy adults. Fasting and postprandial plasma glucose levels, glycosilated hemoglobine (HbA1C) and soluble intercellular adhesion molecule-1 (sICAM-1) levels were measured in all patients. Results: sICAM-1 levels were statistically significantly higher in diabetic patients when compared with control group (500.986±134.35 ng/ml versus 404.316±104.90 ng/ml, p<0.01). When groupped according to the presence of retinopathy the patients with retinopathy had higher sICAM-1 levels than the patients without retinopathy, and this difference was statistically significant (565.17±117.20 ng/ml vs 431.45±117.56 ng/ml, p<0.01). When compared with nonproliferative retinopathy, proliferative rethinopathy had the highest sICAM-1 level (624.51±114.23 vs 495.94± 78.02 ng/ml, p<0.05) Conclusion: Adhesion molecules may have role in the development of microvascular complications of diabetes mellitus. In order to explain their role, prospective follow up studies made in large population is needed
