Özofagus varis eradikasyon tedavisinin fundal varis oluşumu ve şiddetlenmesi ile ilişkisi; retroskopik çalışma

Özet
Amaç: Özofagus varis eradikasyonu sonrasında portal sistemde hemodinamik bazı değişiklikler oluşmaktadır. Bu çalışmada eradikasyonun fundal varis ve portal hipertansif gastropati oluşumu ve şiddetlenmesi üzerindeki etkilerini değerlendirmeyi amaçladık. Gereç ve Yöntem: 1996-2003 yılları arasında kliniğimizce özofagus varis eradikasyon programına alınmış olan hastaların kayıtları retrospektif olarak incelendi ve varisleri eradike olan 89 hastanın eradikasyon öncesi ve sonrası fundal varis ve portal hipertansif gastropati verileri karşılaştırıldı. İstatistik değerlendirme için McNemar testi kullanıldı. Bulgular: Hastaların eradikasyon öncesinde 39 (%43.8)'unda fundal varis varken eradikasyon sonrasında 47 (%52.8)'sinde fundal varis vardı (p<0.05). Eradikasyon öncesinde 60 (%67.4) hastada portal hipertansif gastropati varken eradikasyon sonrası 74 (%83.1) hastada portal hipertansif gastropati vardı (p<0.05). Sonuç: Özofagus varislerinin endoskopik band ligasyonu ile eradikasyonu sonrasında fundal varis ve portal hipertansif gastropati insidansda artış olmaktadır.
Abstract
Objective: After variceal eradication, the hemodynamics of the portal system undergoes changes. The aim of this sudy is to evaluate the effects of eradication therapy on the occurence and severity of fundal varices and portal hypertensive gastropathy. Materials and Methods: Records of 89 patients undergoing eradication therapy in our clinic between 1996 and 2003 were examined and data on fundal varices and portal hypertensive gasropathy before and after eradication were compared. For statistical analysis the Mc. Nemar test was performed. Results: Before eradication, 39 patients (43.8%) had fundal varices, compared to 47 patients (52.8%) who had fundal varices after eradication (P<0.05). Before eradication, 60 patients (67.4%) had portal hypertensive gastropathy, while 74 patients (83.1%) had portal hypertensive gastropathy after eradication (P<0.05). Conclusion: After eradication of esophageal varices by band ligation, fundal varices and portal hypertensive gastropathy occur with increased incidence.
