Özet
Amaç: Nadir olmakla birlikte karşımıza çıkan primer koledok taşlarının sağaltımında gelişen tedavi seçeneklerinin klinik yansımalarının kendi serimizde incelenmesi amaçlanmıştır. Gereç ve Yöntem: Ankara Numune Eğitim ve Araştırma Hastanesi 4. Genel Cerrahi Kliniği'nde Ocak 1992 ile Aralık 2002 tarihleri arasında koledokolitiazis nedeniyle takip ve tedavi edilen 110 hasta retrospektif olarak incelenmiştir. Bulgular: Primer koledok taşı için tanımlanan kriterlere uyan 15 olgu 12 (%80) kadın ve 3 (%20) erkekten oluşmaktadır. Yaş ortalaması 61 (42-80) olarak bulunmuştur. 1992-1996 yılları arasında tedavi edilen tüm olgularda tercih edilen tedavi yaklaşımı koledokoduodenostomi olurken 1997 yılı sonrasında, uygun olan bütün olgular, endoskopik sfinkterotomi ile sağaltılmışlardır. Sonuç: Primer koledok taşlarının tedavisinde öncelikli hedef taşın çıkarılması ve yeniden oluşmasının engellenmesidir. Taşın yeniden oluşumunun engellenmesi için safra akışının devamlılığı sağlanmalıdır. Günümüz şartlarında, tercih edilen yaklaşım endoskopik taş ekstraksiyonu ve sfinkterotomi olmalıdır. Teknik olarak cerrahi yaklaşımın gerekmesi halinde uygun işlem koledokoduodenostomi olmalıdır.
Abstract
Objective: Although incidence is rare for primary common bile duct stones, our study was designed to expose the clinical reflections of the evolving therapy options in the treatment of primary choledocholithiasis. Material and Method: A total of 110 patients with choledocholithiasis which were hospitalized at the 4th Surgical Department of Ankara Numune Education and Research Hospital between January 1992 and December 2002 were retrospectively reviewed. Results: The fifteen patients that met the criterion for the primary common bile duct stones were reviewed. There were 12 (%80) female and 3 (%20) male patients. The mean age was 61 (range 42 to 80). While choledochoduodenostomy had been the procedure of choice in the treatment of all patients between 1992 and 1996, all suitable patients hospitalized after 1997 were treated by endoscopic sphincterotomy. Conclusion: Extraction of the stone and the prevention of the recurrence are the main objectives in the treatment of primary common bile duct stones. Maintenance of the bile drainage is necessary in order to prevent the recurrence. Today, the treatment of choice should be endoscopic stone extraction and sphincterotomy. When surgical approaches are necessitated due to technical limitations, choledochoduodenostomy should be considered.
