Kronik hepatitli hastalarda interferon tedavisinin glukoz toleransına etkisi

Özet
Amaç: Kronik hepatitlerin tedavisinde İnterferon kullanımının yaygınlaşması, interferonun yan etkilerini konu alan çalışmaları gündeme getirmiştir. Amacımız; interferonun glukoz toleransına etkisini araştırarak bu konuya dikkat çekmektir. Gereç ve Yöntem: Histolojik ve serolojik olarak kronik hepatit olduğu ispatlanmış İnterferon tedavisi alan kronik hepatitli 30 hastada açlık kan şekeri ve oral glukoz tolerans testi (OGTT) sonuçları araştırılmıştır. Bulgular: İnterferon tedavisi öncesi iki hastada bozulmuş glukoz toleransı saptandı. Haftada 3 gün uygulanan alfa-interferon (IFN) tedavisinin 4. haftası sonunda, 6 hastada (%20.0) bozulmuş glukoz toleransı, 5 hastada (%16.7) diabetik patern saptandı. Verilerin istatistiksel değerlendirmeleri sonucunda açlık kan şekeri düzeylerinde anlamlı bir değişiklik saptanmazken (p>0.05); OGTT sonuçlarında anlamlı artış (p<0.001) belirlendi. Sonuç: İnterferon tedavisi verilen kronik hepatitli hastalarda, açlık kan şekeri ve OGTT düzeylerinin takibi gerektiği kanaatine varıldı.
Abstract
Objective: in recent years, İnterferon is used in treatment of chronic hepatitis and the studies about the side effects of İnterferon are planned frequently. The effect of İnterferon to glucose tolerance is not wellknown and not paid attention. in this study we aimed to search the effect of İnterferon to glucose tolerance. Material and Methods: in this study we chosed thirty patients who are histologically and serologically proved as chronic hepatitis. Before the treatment fasting blood glucose level and oral glucose tests have done. Result: 2 patients had abnormal glucose tolerance tests before the intereferon treatment. Interferon-alpha treatment has been approved 3 times a week and at the 4th week of treatment fasting blood glucose level and oral glucose tests were repeated. in 6 patients (%20) abnormal glucose tolerance test, in 5 patients (%16.7) diabetic pattern was shovvn. According to the istatistically outcomes; there is no meaningful changes in fasting blood glucose levels (p>0.05) hovvever oral glucose tolerance tests impaired significantly (p<0.001). in chronic hepatitis patients İnterferon therapy alters glucose tolerance. Conclusion: Chrpnic hepatitis patients must be pursuited closely for fasting glucose level and oral glucose tolerance test before and during İnterferon treatment.
