Özet
Amaç: Otonom nöropati, kronik üreminin bir komplikasyonu olarak kronik böbrek yetmezlikli hastalarda da bildirilmiştir. Bu çalışmada amacımız programlı hemodiyaliz tedavisiyle izlenen hastalarda otonom nöropati prevalansını araştırmaktır. Gereç ve Yöntem: Yaşları 17-75 (ortalama 45) arasında değişen 18 hasta çalışmaya alındı. Değişik nedenlerle kronik böbrek yetmezliği gelişmiş olan hastalar 6ay-8yıl (ortalama 3.2 yıl) programlı hemodiyaliz tedavisinde izlenmekteydi. Bu olgularda otonom nöropati test sonuçlan yaşları 21-62 arasında olan (ortalama 42.1) 18 sağlıklı kontrol grubu verileriyle karşılaştırıldı. Parasempatik fonksiyon için valsalva manevrasına kalp hızı yanıtı, zorlu solunuma kalp hızı yanıtı, ayağa kalkmaya kalp hızı yanıtı testleri, sempatik aktivite için istirahatte kalp hızı ve ortostatik hipotansyon değerlendirildi. Bulgular: incelenen 18 hastanın 15'inde (%83.3), 18 kontrolün 5'inde (%27.7) otonom nöropati saptandı (p 0.05). Sonuç: Bulgularımız programlı hemodiyaliz tedavisindeki hastalarda özellikle parasempatik disfonksiyonla karakterize otonom nöropatinin yüksek oranda bulunduğunu göstermiştir. Disotonomi bulgularının erken dönemde belirlenmesinin yüksek riskli bu hastalardaki morbiditeyi ve mortaliteyi azaltabileceği düşünülmüştür.
Abstract
Objective: Autonomic neuropathy is reported as a complication of chronic uremia in patients with chronic renal failure (CRF). in this study, our aim is to investigate the prevalence of autonomic neuropathy in patients under regular haemodialysis treatment. Material and Methods: Eighteen patients aged 17-75 (average 45) were included to the study. Patients with CRF due tö different causes had been follvved 6 months-8 years under programmed haemodialysis treatment. Our patient's findings from autonomic neuropathy tests were compared with the findings of control group aged 21-62' (average 42.1). For parasympathetic functions; heart rate response to Valsalva manoeuvre, heart rate response to deep breathing, heart rate response to standing and for sympathetic function; blood pressure fail on standing, heart rate variation during the rest were assessed. Results: Autonomic neuropathy was determined in 15 of 18 patients included the study (83.3%) and 5 of 18 controls '(27.7%) (p<0.005). Ali of the 15 patients with autonomic neuropathy had parasympathetic dysfunction. Nine of-them had sympathetic dysfunction. So they were assessed combined dysfunction. VVhiie isolated parasympathetic dysfunction was determining in 6 patients. No patient had isolated sympathetic dysfunction. According to the patients' ages, autonomic neuropathy findings were seen in. 10 of 11 patients aged över 45 years and in 5 of 7 patients aged under 45 years. When the age groups were compared, autonomic neuropathy frequency had not statistical significance (p>0.05) Conclusion: Our results have shown that, automatic neuropathy especially parasympathetic dysfunction findings are of high in the patients under haemodialysis treatment. It has thought that determining dysautonomia findings in early period decreases morbidity and mortality in these patients with high risk.
