Özet
Makromastili kanserlerde kanserli dokuların aynı anda memeyi küçülterek çıkarılmasını sağlayan onkoplastik teknikler ve geç sonuçları henüz yeterince bilinmemektedir. Çalışmamızda bu yöntemle tedavi edilen olgularda geç sonuçlar bildirilmektedir. GEREÇ VE YÖNTEM: 1993-2004 arasında kliniğimizde meme küçültme mamoplastisi uygulanan 116 makromastili meme kanseri olgusu evrelerine göre tedavi edilerek izlendi. (Her olguda radyoterapi rutin olarak uygulandı. Adjuvant kemoterapi ve hormonoterapi histopatolojik veriler ve reseptör sonuçlarına göre standart endikasyonlarıyla uygulandı.) Kozmetik sonuçlar, yerel yineleme, metastaz gelişimi ve sağkalım yönünden geriye dönük değerlendirildi. BULGULAR: 10 yıllık yerel yineleme oranları evre I‘de %0 evre II’de % 10.9 ve evre III’de 19.2 oldu. 10 yıllık hastalıksız yaşam oranları evre I’de %100, evre II’de %89.1 ve evre III’de %46.1 oldu. 10 yıllık genel sağkalım oranları ise evre I’de %100 evre II’de %89.1 ve evre III ‘de %53.8 oldu. Kozmetik sonuçlar %84 olguda iyi ve çok iyi idi. SONUÇ: Bu sonuçlar makromastili kanserlerde küçültme ameliyatlarının standart yöntemler kadar etkili ve güvenli olduğunu düşündürmektedir.
Abstract
The oncoplastic techniques those are helpful in the reduction of the breast cancer with macromastia and the removing of malign tumor at the same time and their late results are not well known, yet. In our study, we have reported the late results of the 116 patients with breast cancer treated with this technique. MATERIAL AND METHOD: The reduction mammaplasty and ipsilateral axillary dissection was performed in 116 patients with breast cancer with macromastia.Necessary Adjuvant therapies was applied according to the stage of the cancer.(Chemotherapy and Hormonotherapy was applied with standard indications according to histopathological data and receptor status.Radiotherapy was applied to all cases regularly.)We have treated and followed up the patients between 1993 and 2004, regarding the stage of the disease. The cosmetic results, local recurrence rates, disease free and overall survival were analysed, retrospectively. RESULTS: The local recurrence rates were 0% for stage I, 10,9% for stage II and 19,2% for stage III. Regarding the stage of the disease, 10-year disease free survival rates were 100% in stage I, 89,1% in stage II and 46,1% in stage III patients respectively. The overall survival rates were determined as 100% for stage I, 89,1% for stage II and 53,8% for stage III. The cosmetic results of 85% of patients were good to excellent. CONCLUSION: Our results indicates that breast reduction operations are as effective and safe as standard surgical procedures in patients with macromastia and breast cancer.
