Mastektomide silikon drenler üstün mü?

Özet
AMAÇ: Mastektomi sonrası silikon hemovak kullanımının konvansiyonel hemovak kullanımına göre avantajlı olup olmadığını saptamak. MATERYAL VE METOD: Kliniğimizde Mart 2002-Aralık 2003 tarihleri arasında opere edilen 64 meme karsinomlu hasta çalışmaya alındı. 1. gruptaki 34 hastada konvansiyonel dren, 2. gruptaki 30 hastaya ise silikon dren kulanıldı. Gruplar drenaj miktarı, drenaj süresi, flep nekrozu, yara enfeksiyonu ve seroma oluşumu yönünden karşılaştırıldı. Hemovaklar her iki grupta getiri 30 cc altında iken çekildi. BULGULAR: 1. grupta drenaj süresi ortalama 6.7±3 gün iken, 2. grupta 6.6±2 gün olarak bulundu. 1. grupta ortalama drenaj miktarı 111±33 ml iken, 2. grupta 110±30 olarak bulundu. Her iki parametre açısından iki grupta da istatistiksel anlamlı fark saptanmadı. Her iki grupta da flep nekrozu ve flep enfeksiyonu görülmedi. 1. grupta iki hastada hemovak varken seroma gözlendi. 1. grupta 10 2. grupta 5 hastada seroma dren çekilimi sonrası sadece 1. gün saptandı (p<0.05). SONUÇ: Drenaj süresi ve miktarı ile flep nekrozu ve yara enfeksiyonu göz önüne alındığında silikon drenlerin istatistiksel anlamlı bir avantajları olmadığı saptanmıştır. Ancak muhtemelen drenaj süresince tıkanmadıkları için erken çekilmesi gerekmediğinden seroma oluşumunu azalttıkları düşünülmektedir.
Abstract
AIM: To investigate the advantages of using silicone haemovac drains over conventional ones after mastectomy. MATERIAL and METHOD: Sixtyfour breast carcinoma patients were included in the study operated between March 2002- December 2003. For the 34 patients of fi rst group conventional drains were used whereas for the 30 patients of the second group silicone drains were used. Groups were compared in respect to amount of drainage, duration of drainage, fl ap necrosis, wound infection and seroma collection. Drains were removed when daily drainage drops below 30 cc in both groups. FINDINGS: The mean duration of drainage in the first group was 6.7±3 days, whereas it was 6.6±2 days in the second group. The mean amount of drainage in the both groups was 111±33 cc and 110±30 cc respectively. There was no statistically significant difference between two groups for the amount of drainage and duration of drainage. There was no flap infection nor flap necrosis in both groups. Seroma collection on the first day after drain removal was noted in 10 patients of first group and in 5 patients of second group (p<0.05).
