16 yaşında bir bayan hastada memenin metaplastik karsinomu

Özet
Memenin metaplastik karsinomu tüm meme kanserlerinin %1’inden azını oluşturur. Sıklıkla kötü diferansiye duktal karsinomlarla birliktedir. Bu yazıda 16 yaşında bayan hastada memenin metaplastik karsinomu olgusu ve tedavisi sunulmuştur. Yaklaşık bir yıllık bir sürede giderek büyüyen memedeki kitlenin ultrasonografi ile incelenmesinde; sol memenin hemen hemen tamamını dolduran 6x7 cm boyutlarında düzgün konturlu, heterojen, hipoekojen kitle saptanmıştır. Eksizyonel biopsi sonrası dış merkezde miksioid liposarkom tanısı alan hastanın preparatları hastanemizde yeniden incelendi ve tanı “memenin metaplastik karsinomu” olarak bildirildi. Eksizyon bölgesinde hematom da saptanan hastaya modifiye radikal mastektomi ve aynı seansta latismus dorsi kas deri flebi ile rekonstruksiyon uygulandı. Postoperatif patolojik incelemede, ilave iki ayrı odakta benzer özelliklerde tümöral gelişim saptandı. Östrojen reseptörü negatif, progesteron reseptörü pozitif, C erbB-2 negatif olarak değerlendirildi. Aksilla diseksiyonunda metastaz saptanmayan hastaya cisplatin, doksorubisin ve siklofosfamid’den oluşan 4 kür kemoterapi ve radyoterapi uygulandı. Hasta postoperatif sekizinci ayında sorunsuz olarak takip edilmektedir.
Abstract
Metaplastic breast carcinoma accounts less than 1% of breast malignincies and frequently seen with poorly diff erentiated ductal carcinoma of the breast. In this paper, the clinical findings of a 16 years old female patient with metaplastic breast cancer and the treatment given has been presented. Progressively enlarging mass lesion of left breast was evaluated with ultrasonography. The lesion was 6x7 cm in size with regular borders, heterogenous and hypoechoic structure. Before referral to our hospital, the lesion had been excised at another institution and histopathological diagnosis reported was myxoid liposarcoma. But, the histopathologic diagnosis changed to metaplastic breast carcinoma with the help of immunohistochemistry at our pathology department. The patient was operated with modified radical mastectomy and myocutaneous latissimus dorsi flap was used for immediate breast reconstruction. There was two more foci of the tumor with the similar histopathological morphology. Estrogen receptor and C erbB-2 were negative and progesteron receptor was positive. There was no metastasis to axillary lymph nodes. She was given combination chemotheraphy (cysplatine, doxorubicine and cyclophosphamide) and radiotheraphy and under follow-up without any recurrence for eight months.
