Posterior enstrümantasyon ve posterolateral dekompresyon ile tedavi edilen torakolomber bölge burst kırıklı hastalarda klinik sonuçların değerlendirilmesi

Özet
Bu çalışmada, torakolomber omurga burst kırığı nedeni ile posterior teknikle posterolateral dekompresyon, posterior enstrümantasyon ve füzyon yapılan olgularımızın klinik sonuçlarını değerlendirmesi amaçlanmıştır. Kliniğimizde 1997-2001 yılları arasında posterior stabilizasyon yöntemi ile tedavi edilen 185 olgunun 97 tanesi posterior yaklaşım ile posterolateral dekompresyon, enstrümantasyon ve füzyon uygulanmıştır. Bu olgulardan torakolomber bölgede olan 75’ i çalışmaya dahil edildi. Kırıklar Denis’ e göre sınıflandırılmıştır. Olguların nörolojik durumları Frankel sınıflamasına göre değerlendirilmiştir. Hastaların ortalama takip süresi 61,5 aydır. Olguların ameliyat öncesi ve ameliyat sonrası ve son kontrol değerlendirilmesinde Denis ağrı ve iş skalası kullanılmıştır. Bu skalanın sagittal indeks değeri de göz önüne alınarak yapılan puanlama sonucunda çok iyi (%28), iyi (%45,3), orta (%20), kötü (%6,6) olduğu belirlenmiştir. Bu çalışmada hastaların %73,3’ünde çok iyi-iyi son takip sonu elde edilmesi, az sayıdaki komplikasyon ve tüm hastalarda füzyon gelişimi saptanması nedeniyle iyi seçilmiş olgularda, torakolomber bileşke omurga burst kırıklarının tedavisinde, posterior girişimle posterolateral dekompresyon ve posterior enstrümantasyon ve füzyon uygulamasının etkin ve başarılı bir tedavi yöntemi olduğu inancı elde edilmiştir.
Abstract
This study has been done to evaluate the results of posterolateral decompression, posterior stabilization and fusion in patients with burst fractures of thoracolumbar spine complicated with progressive neurologic deficit. In between 1997-2001, 185 patients were treated with posterior stabilization. In 97 out of 185 posterolateral decompression was combined with posterior stabilization and fusion. This study has been done to evaluate the results in 75 out of 97 patients in thoracolumbar region. Follow up was at least two years with a mean of 61.5 months. Frankel neurologic staging and Denis pain, work scale were used pre- and postoperatively. We evaluated that Frankel neurologic staging, Denis pain, work scale and sagittal index score. It’s excellent (%28), good (%45,3), poor (%20), fair (%6,6). In the existence of progressive neurologic deficit, surgical management of the thoracolumbar burst fractures should cover decompression, post instrumantation and fusion. In conclusion, posterolateral decompression approach is successful if posterior elements of the spine disperts the medulla spinalis.
