Konjenital skolyozlu olgulardaki kot anomalilerinin posterior enstrümantasyon ile yapılan korreksiyon üzerine etkisi

Özet

Amaç: Konjenital skolyozda sık karşılaşılabilecek anomalilerden biri de eğriliğin konkav tarafındaki kot anomalileridir. Eğriliğin konkav kısmında bulunan kot anomalileri ve füzyonları eğrilik korreksiyonunu ve idamesini olumsuz yönde etkileyebilmektedir. Bu çalışmadaki amacımız kot anomalilerinin korreksiyon üzerindeki etkilerinin araştırılmasıdır. Yöntem: 1995 - 2008 yılları arasında kliniğimizde konjenital skolyoz tanısıyla opere edilmiş olan 42 olgu (13 erkek, 29 kız) retrospektif olarak incelendi. Hastaların ortalama yaşı 11 (2-34 yaş), ortalama takip süreleri 63 aydı (6-144 ay). Kot anomalilerini çoğunlukla vertebra anomalisiyle aynı seviyelerde görülen kot füzyonları oluşturuyordu. Cerrahi yöntem olarak 16 hastaya hemivertebra eksizyonu ile birlikte konveks taraf hemiepifizyodez, 1 hastaya hemivertebra eksizyonu ve posterior füzyon, 5 hastaya kombine anterior ve posterior füzyon,12 hastaya posterior füzyon, 5 hastaya posterior girişim ile hemiepifizyodez ve hemiartrodez, 3 hastaya füzyonsuz posterior implantasyon yapıldı. 18 hasta tek seansta opere edilirken, 24 hastaya ek cerrahiler uygulandı. Bulgular: Hastalarda bulunan majör eğrilikteki düzelme % 35.3 olarak saptanırken, kompansatuar eğrilikteki düzelme % 39 olarak tespit edildi. Global kifoz ve lordoz açılarında önemli derecede bir değişiklik saptanmadı. Kifoskolyozu bulunan 6 hastanın cerrahi sonrası preoperatif kifoz açıları ortalama 63º’den (42º-93º), ortalama 46º’ye (19º-83º) düştü. Bu olgularda korreksiyon miktarı % 26 idi. Kot anomalisi bulunan olgularda ana eğrilikte düzelme % 23 olarak belirlenirken, kot anomalisi bulunmayan olgularda saptanan ana eğrilikteki % 44’lük bir düzelme istatistiksel olarak anlamlı bulundu Sonuçlar: Cerrahi tedavi gören, eğriliğin konkav tarafında kot anomalileri bulunan konjenital skolyoz olgularında korreksiyonun daha az bulunması cerrahın göz önünde bulundurması gereken bir faktördür. Bu tip olgularda birleşmiş kotların en blok rezeksiyonu yada ekspansiyon torakoplasti ve kaburga titanyum prostetik vertikal genişletici (vertical expansion prosthetic titanium rib - VEPTR) cihazının yerleştirilmesi korreksiyon oranlarını arttırabilir.

Abstract

Purpose: Rib abnormality at concavity of the curve is one of the frequently seen anomalies in congenital scoliosis. These rib anomalies and fusions at the concave site can negatively effect the correction and maintenance of the curve. The aim of this study is to detect the effects of rib anomalies on correction. Method: 42 congenital scoliosis patients (13 male, 29 female) who had surgery at our hospital between 1995 and 2008 were retrospectively evaluated. The mean age was 11 (2-34), and mean follow up period was 63 months (6-144 months). Most of the rib anomalies were fusions that were seen at the same level of the vertebral anomalies. The surgical methods were; hemivertebrectomy with hemiepiphysiodesis at concave site in 16 patients, hemivertebrectomy with posterior fusion in 1 patients, combined anterior and posterior fusion in 5 patients, posterior fusion in 12 patients, posterior hemiepiphysiodesis and hemiarthrodesis in 5 patients and posterior instrumentation without fusion in 3 patients. Whereas 18 patients were operated at one stage, 24 patients had additional surgeries. Findings: When the correction of the major curves were determined as 35.3 %, the correction of the compensatory curves were determined as 39 %. There were no changes in the global kyphosis and lordosis angles. The preoperative kyphosis angles were decreased to mean 46 º (19º-83º) from mean 63º (42º-93º) after surgery in 6 kyphoscoliosis patients. The correction rate was 26 % in these patients. The correction rate at the main curve was determined as 23 % in patients with rib anomalies, whereas it was determined as 44 % in patients without rib anomalies, that it was detected statistically significant (p<0.005). Conclusion: Less correction rates after surgery in congenital scoliosis patients who have rib anomalies at concave site of the curve is an important factor that surgeons must decide on it. En bloc resection or expansion thoracoplasty and to place vertical expansion prosthetic titanium rib &#8211; VEPTR can raise correction rates in these patients.
