Özet
Omurga enfeksiyonları, vertebra cisimlerini, intervertebral diskleri, spinal kanalı ve paravertebral yumuflak dokuları etkiler. Çok sık görülmemekle beraber tanı koymadaki zorluklar tanının gecikmesine ve çok ciddi sonuçların ortaya çıkmasına yol açmaktadır. Gecikmifl tanı omurga deformitesine, önemli nörolojik komplikasyonlara hatta ölüme sebep olabilir. Yoğun çalışlmalara rağmen omurga enfeksiyonlarının sınıflandırılması, tanısı ve tedavisinde hala karışıklıklar söz konusudur . Osteomiyelitin ortaya çıkışı hastanın yaşı, immün cevabı ve etken mikroorganizmaya bağlıdır. Uygun şartlar altında hemen hemen tüm enfektif organizmalar vertebral enfeksiyona yol açabilirler. Piyojenik formda en sık etken Staphylococcus aureus’ tur. Bunun dışında koagulaz negatif stafilokoklar, Enterobacteriaceae ve diğer Gram-negatif basiller, aerob Gram-negatif basiller (Pseudomonas aeruginosa gibi), hemolitik streptokoklar, Haemophilus influenzae, Streptococcus pyogenes, Salmonella türleri tarafından meydana gelir. Omurga enfeksiyonları, ciddi kas-iskelet ve nörolojik komplikasyonların engellenebilmesi için erken evrede tanı konması gereken ağır bir patolojik durumdur. Klinik bulgular açısından enfeksiyondan şüphelenildiği anda antimikrobiyal tedavinin en iyi şekilde yapılabilmesi için mikrobiyolojik kültür ve duyarlılık testleri uygulanmalıdır. Mikrobiyoloji teşhiste altın standarttır ve tanıyı doğrulamak için mutlaka gereklidir.
Abstract
Spinal infections affect the vertebral bodies, the intervertebral disks, the spinal canal, and the paravertebral soft tissues. A delay in diagnosis can result in spine deformity, substantial neurologic complications, and even death. The presentation of osteomyelitis depends on the age of the patient, his immune response and the infecting organisms. Almost any infective organism can, in suitable conditions, cause vertebral infection. In the pyogenic form of the disease, the most common organism is Staphylococcus aureus. Other organisms include coagulase-negative staphylococci, Enterobacteriaceae and other Gram-negative bacilli, aerobic Gram-negative bacilli (such as Pseudomonas aeruginosa), haemolytic streptococci, Haemophilus influenzae, Streptococcus pyogenes, Salmonella spp. Spinal infections represent severe pathologic conditions that should be diagnosed in an early stage, avoiding serious musculoskeletal and neurologic complications. Microbiological culture and antimicrobial susceptibility testing should be employed for the successful treatment as soon as infection is clinically suspected. Microbiologic diagnosis is the gold standard and has the key role to confirm the diagnosis.
