Osteoporotik vertebra kompresyon kırıklarının ekstrapediküler vertebroplasti ile tedavisi

Özet
Amaç: Osteoporotik vertebra kompresyon kırıklarında ekstrapediküler vertebroplasti ile tedavi sonuçlarının değerlendirilmesi. Materyal ve Metot: Osteoporotik vertebra kompresyon kırığı tanısıyla ekstrapediküler vertebroplasti uygulanan, bir yıl ve üzerinde takip edilen 47 hasta çalışmaya alınmıştır. Hastaların 29’u (% 61,7) kadın, 18’i (% 38,3) erkek olup, yaş ortalaması 66,9 idi (38-87). Toplam 74 seviyeye vertebroplasti uygulanmıştır. Hastaların ağrı skorları, analjezik gereksinimi ve mobilizasyon kapasitesi değerlendirilmiştir. Bulgular: VAS, analjezik gereksinimi, mobilizasyon kapasitesinde ameliyat sonrası erken dönemde başlayan ve bir yıllık takipte devam eden anlamlı iyileşme saptanmıştır (p< 0,001). Yapılan işlemlerle ilgili herhangi bir komplikasyon görülmemiştir. Sonuç: Perkütan ekstrapediküler vertebroplastinin osteoporotik vertebra kompresyon kırıklarının tedavisinde transpediküler yönteme alternatif, güvenli ve etkin bir yöntem olduğu sonucuna varılmıştır.
Abstract
Purpose: Evaluation of extrapedicular vertebroplasty in the treatment of osteoporotic vertebral compression fractures. Materials and Methods: Forty seven patients (29 female, 18 male) who underwent extrapedicular vertebroplasty for osteoporotic vertebral compression fractures with a minimum follow-up of one year were included in the study . Average age was 66.9 years (range 38-87). 74 levels were operated. Pain scores, analgesic need and mobilization capacity were evaluated. Results: Significant improvement was observed in VAS, analgesic need, and mobilization capacity at the early postoperative period that persisted at one year follow-up (p< 0,001). No complication related to the procedure occurred. Conclusion: Percutaneous extrapedicular vertebroplasty is a safe and effective method, alternative to transpedicular vertebroplasty in the treatment of osteoporotic vertebral compression fractures.
