Vertebroplasti, intraoperatif sistemik komplikasyonlar önlenebilir mi?

Özet
Amaç: Perkütan vertebroplasti uyguladığımız hastalarda, anestezi monitorizasyon değişikliklerini incelemek ve ameliyat esnasında gelişebilecek dahili komplikasyonları önleyebilmek için yapılması gerekenleri gözden geçirmek. Hastalar ve Yöntem: Ekim 2001 ve Ağustos 2007 tarihleri arasında kliniğimizde perkütan vertebroplasti uygulanan hastaların verileri geriye dönük olarak incelendi. Çalışma grubunu yaş ortalaması 68.2 olan 40 hasta (24 kadın, 16 erkek) oluşturdu. Hastaların büyük bölümüne (%92.5) lokal anestezi ve sedasyon eşliğinde perkütan vertebroplasti uygulandı. Cerrahi işlem sırasındaki anestezi monitorizasyon verileri incelendi ve olası komplikasyonlara karşı alınabilecek önlemler değerlendirildi. Sonuçlar: Torakolomber bölgenin sık etkilendiği (% 62.5) ve osteoporotik kırık (% 80) etyolojisinin fazla görüldüğü çalışma grubunda, 5 olguda cerrahi işlem sırasında ani hipotansiyon değişiklikleri gözlendi. İki olguda tansiyon iniş-çıkışları ve 1 olguda dispne gözlendi. Ameliyat sonrası dönemde çekilen bilgisayarlı tomografide 22 olguda asemptomatik sement kaçağı tespit edildi. Çıkarımlar: Komplikasyonlar en sık sement kaçağına bağlı görülmektedir. Sement kaçağını önlemek için uygun cerrahi teknik uygulanmalı ve hasta hidrasyonu iyi sağlanmalıdır. Mevcut kardiyak ve pulmoner hastalıkların tedavisi ameliyat öncesinde başlanmalıdır. Cerrahi işlem sırasında anestezist, komplikasyonlarla baş etmeye hazırlıklı olmalıdır.
Abstract
Aim: To evaluate the anesthesia monitor changes and to review the possibilities to prevent Intraoperative medical complications in the percutaneous vertebroplasty applied patient group. Patients and Methods: The data of the percutaneous vertebroplasty applied patients, between October 2001 and August 2007, were retrospectively evaluated. The study group included 40 patients (24 women, 16 men) with a mean age of 68.2. Most of the patients (92.5 %) were operated under local anesthesia and sedation. Anesthesia monitor changes during the operation were evaluated and the preventions against the possible complications were reviewed. Results: In the study group, which was mostly formed by thoracolumbar (62.5 %) and osteoporotic fractures (80 %), hypotansive attacks during percutaneous vertebroplasty were observed in 5 patients. Two patients had tension irregularities and 1 patient had dyspne. In the postoperative computerized tomography scans, 22 asymptomatic cement leakages were observed. Conclusions: Most of the complications are the result of cement leakage. In order to prevent cement leakage, appropriate surgical technique should be applied and the patient must be well hydrated. The treatment regimen for the existing cardiac and pulmonary diseases should be put into order preoperatively. During the operation, anesthetist must be ready to manage the probable complications.
