Özet

Amaç: Ailevi Akdeniz Ateşi (AAA) geni 16p kromozom üzerinde bulunmakta ve 50’den fazla mutasyon içermektedir. Etnik ve çevresel faktörler mutasyonun fenotipik etkilerini değiştirebilmektedir. Bu çalışmadaki amacımız, hastalarımızdaki genotip ve fenotip ilişkisini ortaya koymaktır. Yöntem: Çalışmamıza Çocuk Nefroloji Polikliniğinden Tel-Hashomer kriterlerine göre tanı almış 81 AAA hastasının dosyalarından geriye dönük olarak yapıldı. Tüm hastaların mutasyon sonuçları ile fenotipik özellikleri, ağırlık skorlaması, tedavi yanıtı arasında ilişkiler değerlendirildi. Hastalık ciddiyet skorlaması PRAS skorlamasına göre yapıldı. Bulgular: Hastalarımızın tanı anındaki yaşlarının ortalaması (±SD) 9,2±3,4 yıldı. Hastalarda en sık görülen yakınma karın ağısı (%98,8) ve artralji (%80,2) idi. Hastalık ağırlık derecelerine göre tüm hastaların %32,1’i hafif, %63,0’ı orta ve %4,9’u ağırdı. Elli altı hasta ve 88 allelde 7 farklı MEFV geni saptandı. M694V en sık (%30,2) saptanan mutasyondu. M694V homozigot mutasyon olan hastalarda artrit görülme sıklığı daha fazla idi (p=0,04). Hastalık ağırlıkları açısından mutasyon grupları arasında anlamlı farklılık olmamasına karşın M694V homozigot olanlarda amiloidoz ve ağır hastalık görülme sıklığının daha fazla olduğu görülmüştür. Sonuç: MEVF mutasyonu AAA tanısında destekleyici ve tamamlayıcı bir tanı yöntemidir. Hekimler tedavi ve takip planını klinik bulgular ve AAA mutasyonuna göre karar vermelidir. Özellikle amiloidoz olgularının M694V genotipine sahip hastalarda daha sık görülebildiği unutulmamalıdır.

Abstract

Objective: The Familial Mediterranean Fever (FMF) gene has been cloned to chromosome 16p and more than 50 different mutations have been identified. Ethnic and environmental factors can change the phenotypic effects of mutations. We aimed to evaluate genotype-phenotype correlation in our patients. Methods: Eighty-one children with FMF diagnosis from our Pediatric Nephrology Policlinic based on Tel Hashomer criteria were enrolled in the study. Clinical and genetic features of the patients were retrospectively analyzed. The relationships between results of mutations, phenotypic features of all patients, weight scoring, and treatment response were evaluated. Disease severity was assessed using the modified scoring system of PRAS. Results: The mean (±SD) age of the patients at onset of the diagnosis was 9.2±3.4 years. The most frequent symptoms of the patients were abdominal pain (98.8%) and arthralgia (80.2%). The level of disease severity was mild in 32.1%, moderate in 63%, and severe in 4.9% of the cases. Seven different MEFV genes were found in 56 patients and 88 alleles, and M694V was the most frequent mutation (30.2%). In the patients with homozygous M694V mutation, significantly higher frequency of arthritis was observed (p=0.04). Although there was no statistically significant difference among different mutation groups, higher incidence rates of amyloidosis, and severe disease were observed in patients with homozygous M694V mutations. Conclusion: Detection of MEFV mutation supports, and complements the diagnosis of FMF. . The physicians have to decide the treatment and follow-up plan according to the clinical features and AAA mutations. It should be kept in mind that amyloidosis is seen more frequently in the patients with M694V genotype.
