Özet

Atopik dermatit alerjen, kimyasal madde ve infeksiyonlar gibi çeşitli çevresel tetikle- yicilerin ortaya çıkardığı aşırı cilt duyarlılığı ile ilişkili kronik enflamatuar bir deri hastalığıdır. Atopik dermatit genellikle “atopik marş”ın ilk basamağıdır. Atopik der- matit sistemik bir hastalığın cilt tutulumu olarak karşımıza çıkabilir, sütçocukluğu döneminde gıda alerjisi ve hışıltı atakları ile devam edebilir ve daha sonra astım ve allerjik rinit gelişimine neden olabilir. Ağır atopik dermatit özellikle filagrin gen (FLG) mutasyonu olan ve/veya belirtileri erken yaşta başlayan ve ailesinde atopi öyküsü olan yüksek riskli süt çocuklarında daha sık görülür. Tanı klinik bulgular ile konur, nesnel bir laboratuar testi bulunmamaktadır. Bu nedenlerle, atopik dermati- tin ağırlığının klinik ağırlığın skorlandığı testler yardımı ile tespit edilmesi ileride gelişebilecek alerjik hastalıkların ve olası ölümcül komplikasyonların öngörülmesi için oldukça önemlidir.

Abstract

Atopic dermatitis is a chronic inflammatory skin disease associated with cutaneous hyperreactivity to various environmental triggers including allergens, chemicals and infections. Atopic dermatitis is often the initial step in ‘atopic march’. Atopic derma- titis might be a cutaneous manifestation of a systemic disorder that also continues with food allergy and wheezing episodes in infancy period, and gives rise to asthma and allergic rhinitis. Severe atopic dermatitis is seen particulary in children with filaggrin (FLG) gene mutation and/or early beginning symptoms in high risky infants with a history of familial atopy. The diagnosis is based on the clinical findings, and an objec- tive laboratory test does not exist. Therefore, it is important to determine the severity of atopic dermatitis with a validated clinical severity scoring index to predict both future development of allergic diseases and also possible fatal complications.
