Özet
Giriş: Viral pandemi döneminde COVID-19 tanısı almış hastalarda nötrofil, lenfosit, nötrofil-lenfosit oranı ile hemoglobin düzeylerinin, hastaların mortalite oranları, hastane yatış süreleri ve demografik özellikleri ile ilişkisinin değerlendirilmesi amacıyla yapılan bu çalışmada hepsidin-ferroportin aksının fizyopatolojideki yerinin irdelenmesi amaçlanmıştır. Gereç ve Yöntem: Hastanemiz pandemi servislerine COVID-19 tanısı ile yatan hastalar hastanemiz bilgi sisteminden taranarak hastaların laboratuar ve klinik özellikleri ile demografik bulgular analiz edilmiştir. Parametrelerin karşılaştırılmasında Independent T-test’i, one-way ANOVA ve Chi-Square kullanılıp sayı, korelasyonlar, yüzdelik, oranlar, ortalamalar ve standart sapmalar IBM SPSS Windows ver.24 ile hesaplandı. Bulgular: Çalışmaya 49 erkek ile 50 kadın hasta alınmış olup toplamda 99 hastadan oluşmaktadır. Nötrofillenfosit oranı ile hemoglobin ve yaş parametreleri arasında anlamlı bir ilişki saptandı. Başka bir deyişle yaş artıkça nötrofil-lenfosit oranında artış izlendi (r:0.347, p<0.001). Ayrıca nötrofil-lenfosit oranı artıkça hemoglobin değerlerinde bir azalma izlendi. Başka bir deyişle COVID-19 tanılı hastalarda nötrofil-lenfosit oranı artıkça hemoglobin düzeyleri azalmaktadır (r:0.232, p:0.021). Sonuç: Nötrofil-lenfosit oranı ile hemoglobin düzeyleri arasında negatif bir korelasyon olması hepsidin-ferroportin aksının COVID-19 hastalarında sitokin fırtınası vs. gibi nedenlerle etkilendiğini düşündürmektedir. Bu istatistiksel ilişkinin daha geniş ve uzun süren çalışmalarla aydınlatılması ve nedenlerinin saptanması belki de pandemi döneminde etkin tedavi yöntemlerinin geliştirilmesine neden olacaktır.
Abstract
Introduction: This study was conducted to evaluate the relationship between laboratory and clinic parameters, including neutrophil, lymphocyte, neutrophil-lymphocyte ratio, hemoglobin levels, mortality, length of stay, and demographic characteristics of patients diagnosed with COVID-19 during the viral pandemic. We aimed to examine the role of the hepcidin-ferroportin axis in physiopathology. Materials and Methods: Patients hospitalized in the pandemic services of our hospital with the diagnosis of COVID-19 were scanned from the Probel electronic patient registry system, and then the laboratory parameters, clinical features with demographic findings of the patients were analyzed. In the comparison of the parameters, independent t-test, one-way ANOVA, and Chi-Square were used, and numbers, correlations, percentages, rates, averages, and standard deviations were analyzed with IBM SPSS Windows ver.24. Results: The study included 49 men and 50 women. There was a significant relationship between neutrophillymphocyte ratio with parameters of hemoglobin and age. In other words, as the age increased, an increase in the neutrophil-lymphocyte ratio was observed (r: 0.347, p: 0.000). Besides, as the neutrophil-lymphocyte ratio increased, a decrease in the level of hemoglobin was observed. As the neutrophil-lymphocyte ratio increases in patients diagnosed with COVID-19, the level of hemoglobin decreases (r: 0.232, p: 0.021). Conclusion: The negative correlation between neutrophil-lymphocyte ratio and the level of hemoglobin suggests that the hepcidin-ferroportin axis in patients with COVID-19 was affected by the cytokine storm. The determining of this statistical relationship with larger and longer studies and revealing its causes might lead to the development of effective treatment methods during the pandemic period.
