Özet

AMAÇ: Saçlı Hücreli Lösemiye yaklaşımı ve bu hastalarla ilgili deneyimimizi geriye dönük değerlendirmek. GEREÇ VE YÖNTEM: Son 9 yıl içinde Erişkin Hematoloji Polikliniğinde izlenen hastaların dosyaları taranarak Saçlı Hücreli Lösemi tanısı konan 6 olgunun verileri değerlendirmeye alındı. BULGULAR: Hastaların yaşı 52 ile 83 arasında değişmekteydi ve yaş ortalamaları 66,8 idi. Tüm hastalara tedavi başlandı. Beş hastada ilk tedavi olarak interferon-α seçildi. Interferon-α başlanan olgulardan 3’ü izlemi bıraktı. Diğer 2 hastanın birinde hiç yanıt alınamazken, diğer hastada kısmi yanıt alındı. Bu hastalara ikinci basamak tedavisi olarak 2-klorodeoksiadenozin tedavisi başlandı. Bu tedavi altında interferonla yanıt alınamayan hastada kısmi; kısmi yanıt alınan hastada ise tam yanıt elde edildi. İlk basamak tedavide 2-klorodeoksiadenozin başlanan 1 olguda tam yanıt alındı. Hiçbir hastaya splenektomi veya 2’-deoksikoformisin uygulanmadı. SONUÇ: Saçlı hücreli löseminin gidişi özellikle yaşlı toplumda daha kötüdür ve pürin benzeşikleriyle tedaviye iyi yanıt vermektedir.

Abstract

AIM: To evaluate the management of hairy cell leukemia and search retrospectively our experience with hairy cell leukemia patients. MATERIAL AND METHOD: The files of the patients which were followed up in our adult Hematology center were searched during the last 9 years and the data of 6 patients with hairy cell leukemia were evaluated retrospectively. FINDINGS: The range of the age of the patients was 52-83 years and the mean age was 66,8. Treatment was started to all patients. Interpheron-alpha was chosen as first line treatment for 5 patients. Three of these patients left the clinical management. One of the other two patients had no response and the other one had a partial response. 2-Chlorodeoxyadenosine therapy were started to these patients as second line therapy. The patient who had no response with interpheron-α therapy had partial response; and the other patient who had partial response with interpheron therapy had complete response to chlorodeoxyadenosine. The case who had 2-chlorodeoxyadenosine therapy as first line therapy achieved a complete response. None of the patients had splenectomy or 2’-deoxycoformycin. CONCLUSION: Hairy cell leukemia seems especially in the older population and responds to the purine nucleoside analogs good.
