Lenf düğümü metastazı gösteren jejunal gangliyositik paraganglioma olgusu

Özet
Gangliyositik paragangliomalar (GP) çoğunlukla duodenum ikinci kısmında yerleşen, ender görülen, nadiren bölgesel lenf düğümü metastazı ve yineleme gösterebilen nöroendokrin neoplazilerdir. Karın ağrısı, bulantı, kusma ve kilo kaybı olan 34 yaşında kadın olguda, ince barsak pasaj grafısinde jejunumda tümör ve tümör proksimalinde dilatasyon saptanması üzerine segmental ince barsak rezeksiyonu yapıldı. Histolojik, histokimyasal ve immunohistokimyasal bulgular eşliğinde olguya jejunal yerleşimli, lenf düğümünde metastaz gösteren gangliyositik paragangliyoma tanısı kondu. İlginç yerleşim yeri ve son derece nadir görülen lenf düğümü metastazı nedeniyle sunuldu.
Abstract
Gangliocytic paragangliomas (GP) are uncommon benign neuroendocrine neoplasms, encountered exclusively in the second portion of the duodenum which can rarely recur and metastase to regional lymph nodes. We report a 34-year-old woman presented with abdominal pain, nausea, vomiting and weight loss. In the radiological examination of small intestine the tumor located in the jejunum and caused luminal dilatation proximal segment of jejunum above the mass. The patient underwent segmentary small bowel resection. The patient has been diagnosed as gangliocytic paraganglioma showing lymph node metastasis with histochemical and immunohistochemical properties. We are presenting the case due to the exceptional location and rare lymph node metastasis.
