Prostatektomi sonrası idrar kaçırmalarında transobturator askı operasyonu: 27 olguda sonuçlari

Özet

Amaç: Post-prostatektomik üriner inkontinans nedeniyle uygulanan transobturator askı yönteminin etkinliğinin değerlendi- rilmesi. Gereç ve Yöntem: 2007-2012 yılları arasında prostatektomi operasyonları sonrası gelişen idrar kaçırma nedeniyle 27 hastaya transobturator askı operasyonu uygulandı. Hastaların inkontinans derecesinin tespiti 24 saatte kullandığı ped sayısına göre yapıldı. Günlük 1-2 ped kullanımı hafif, 3-5 ped kullanımı orta ve >5 ped kullanımı ciddi inkontinans olarak sınıflandırıldı. Transobturator askı operasyonu sonrası hastaların takiplerinde idrar inkontinansı olmayan ve ped kullanmayan hastalar kuru, 2 ped/gün kullanımı veya kullanılan ped sayısı <%50 azalan hastalar başarısız olarak kabul edildi. Bulgular: Seride yaş ortalaması 64.3(58-72) dür(Postoperatif 3. ay kontrollerinde hastaların 9’unda (%33,4) tam kuruluk, 7’sinde (%25,9) klinik iyileşme saptandı. 11 hastada (% 40,7) ise operasyon öncesine göre herhangi bir değişiklik olmadığı görüldü. Preoperatif / postoperatif ölçülen abdominal zayıf nokta basıncı (AZNB) değerleri ve preoperatif / postoperatif kul- lanılan günlük ped sayıları arasında istatistiksel olarak anlamlı farklılık saptandı. (p<0,05). Hastaların 12. ay kontrollerinde, retropubik radikal prostatektomi (RRP) sonrası inkontinan olup postoperatif 3. ayda tam kuruluk sağlanan 2 hasta ve klinik iyileşme sağlanan 1 hastada orta derecede inkontinans saptandı. İnkontinans derecesine göre değerlendirildiğinde başarı ora- nının; hafif derece inkontinans grubundaki 5 olgunun tümünde( %100), orta derece inkontinans grubundaki 18 olgunun 8’inde(%44,4) olduğu saptandı. Şiddetli inkontinans grubundaki 4 olgunun tümünde(%100) ise operasyonun yarar sağlama- dığı görüldü. Sonuç: Transobturator askı yöntemi, hafif-orta derecede post-prostatektomik üriner inkontinansı olan seçilmiş olgularda ter- cih edilebilir bir yöntemdir. İnkontinans etiyolojisi radikal prostatektomi olan ve ileri düzey kaçırmalarda bu prosedürün ba- şarı şansının bulunmadığı ortaya konmuştur.

Abstract

Aim: To evaluate the effectiveness of transobturator sling operation on patients with post prostatectomy incontinence. Material and Method: Transobturator sling procedures were performed on 27 incontinent, post prostatectomy patients be- tween 2007 and 2012. The degree of incontinence was determined by the number of pads used daily. 1-2 pads/day, 3-5 pads/day and >5 pads/day were classified as mild, moderate, severe incontinence, respectively. During the followup for the transobturator sling operation; the continent patients with no need for pads, <2 pads/day and less than 50% decrease in the total numbers of pads used daily were classified as ‘dry’, ‘clinical improvement’ and ‘failure’, respectively. Findings: On 3rd postoperative month; 9 patients (33,4%) were completely dry, 7(25,9%) had clinical improvement and 11 patients(40,7%) experienced no difference compared with the preoperative period. There was a statistically significant differ- ence between the preoperative and postoperative ALPP (abdominal leak point pressure) values and pads used daily (p<0,05). On 12th month followup, moderate incontinence was detected on 3 patients with retropubic radical prostatectomy; one with clinical improvement and two being dry on the 3rd postoperative month. When the success rates were evaluated accordingthe level of incontinence; 100%, 44,3% and 0% success rates were observed for mild, moderate and severe incontinence, re- spectively. Conclusion: Transobturator sling method is a reasonable choice for selected post prostatectomy patients with mild-moderate incontinence. It’s noteworthy that patients having radical prostatectomy as the etiologic factor for urinary incontinence, this procedure has lower success rates.
