489 tiroidektomi olgusunun retrospektif analizi

Özet
İAB Sintigrafi ve USG ile değerlendirilmiş tiroidektomi olgularında karsinoma insidansını belirlemek ve tiroid cerrahisindeki gelişmeler ışığında klinik bulgularımızı değerlendirmek amacı ile SSK Göztepe Eğitim Hastanesi I. Cerrahi kliniğinde 1997 Ocak-1999 Temmuz tarihleri arasında opere edilen, Sintigrafi ve USG ile mültinodüler tiroid hiperplazisi saptanan İAB leri yapılmış 489 tiroidektomi olgusu geriye dönük olarak incelendi. Olgularda Kadın/Erkek oranı 394/115 kadınlarda malignite riski 12/394 erkeklerde ise 6/115 idi. Olguların 190 tanesinde soğuk nodul saptanıp bunlardan 18 tanesi de Tiroid Ca tanısı almıştır. En çok uygulanan cerrahi %82 ile BST olup. Mortalite yoktur. İAB ve USG gibi tanı yöntemlerinin kullanılmasının artmasına rağmen karsinoma insidansı %3.68 bulunmuştur. Bu da endemik guatr bölgesi olan ülkemizde tiroidin mültinodüler büyümelerinde yaygın olarak cerrahi yöntemin uygulanması endikasyonlarda kozmetik ve bası belirtilerinin ön planda olması olarak değerlendirilmiştir.
Abstract
On Purpose of the determine the carcinoma incidence at the cases of thyroidectomy which is evaluated with USG, FNA, scintigraphy and also at the purpose of clinical evaluation at the thyroid surgery for 489 thyroidectomy cases were investigated retrospectively which were operated between 97 January -99 July at SSK Göztepe Educational hospital from 1st- surgical clinic the results were women/men ratio 394/115, the malignity risks at the women 12/394, at the men 6/115 at 190 cases we find out cold nodules. From 18 of them thyroid ca diagnosis. Bilateral subtotal thyroidectomy is the most frequent operationing technique and without mortality risk although increasing of usage of the methods FNA, USG carcinoma incidence is 3.62%, it is evaluated in our country where is region of endemic goiter, using surgical techniques at largement indication sign at the cosmetical problems and pressure.
