Özet
Myastenia gravis istemli kaslarda fluktuasyon gösteren güçsüzlükle karakterize otoimmun bir hastalıktır. Temel patofizyolojisi motor son plaktaki postsinaptik asetil kolin reseptörlerine karşı oluşan antikorların iletiyi engellemesidir. Literatürde bu hastalığın diğer otoimmun kaynaklı hastalıklarla birlikteliğine dikkat çekilmiştir. Myastenik hastalarda görülen diğer otoimmun hastalıklar arasında tirotoksikoz, romatoid artrit, Sjögren sendromu, mikst konnektif doku 9hastalığı ve polimyozitin olduğu bildirilmiştir. Yayınlanan bildirilerde, sıklıkla romatoid artrite bağlı D-penisilamin kullanımı sonucu myastenik olgular görüldüğü, ancak ilaç kullanımına bağlı olmayan romatoid artrit-myasteni birlikteliğinin nadir görüldüğü vurgulanmıştır. Sunulan olgumuz bu nadir birlikteliğe bir örnek oluşturmaktadır.
Abstract
Myasthenia gravis is an autoimmune disorder characterized by fluctuating weakness of the voluntary muscles. The main pathophysiology is the interruption of the neuromuscular transmission by the antibodies formatted against the postsynaptic acetylcholine receptors on the motor end plate. In the literature, attention was called to the association of this disease with the other autoimmune disorders. Among other autoimmune diseases that could be seen in myastheni patients are thyrotoxicosis, rheumatoid arthritis, Sjogren syndrome, mixed connective tissue disease and polymyositis. In the published reports, myasthenic cases have often been reported due to the D-peniillamine therapy for rheumatoid arthritis, but the pure association of myasthenia with rheumoatoid arthritis has been informed to be rare. Our presented case is an example for this rare association of myasthenia gravis and rheumatoid arthritis.
