Özet
Daha önce gastrik yakınması olmayan ve son 1,5 aydır halsizlik,iştahsızlık, kilo kaybı; bir hafta önce 2-3 kez tekrarlayan herfıatemez ve melena şikayetleri ile başvuran 60 yaşındaki erkek hastanın.ilk fizik muayenesinde; grade II,diffuz guatr, hipertansiyon ve KOAH tespit edildi. Laboratuar incelemede patolojik bulgu saptanmadı. Anamnez,fizik muayene ve laboratuar bulgularına dayanılarak olası GİS malignite ön tanısı ile yapılan baryumlu mide duodenum grafisinde fundus-korpus bileşkesinde irregüler dolma defekti saptandı. Bu bulgu üzerine istenen gastroskopide 10x5x5 cm ebatlarında fitobezoar tespit edildi.Fitobezoarın gastroskopi sırasında polipektomi sneeri ile parçalana-maması nedeniyle yapılan cerrahi konsültasyonu sonucu gastrotomiye karar verildi. Mide içinde oluşan sert yabancı materyal olarak tanımlanan bezoarlar oldukça seyrek görülen gastrointestinal patolojilerdir.Midenin motor fonksiyon bozuklukları ve inorganik madde yenmesi oluşum sıklığını arttırmaktadır. Gastroskopinin günlük uygulamaya girmesi ile tanınması çok kolay olan bezoarlar nadir görülmesi nedeniyle ilginçtir.
Abstract
The 60 years old male patient which previously hasn't got any gastric compliants applied with weakness,losing weight and having no appetite in last 1,5 months and hematesis and melena repeating 2 or 3 times in last week.In his first physical examination grade II diffuse goitre,hypertension and chronic obstructive lung disease found. There wasn't any pat¬hology in laboratory tests. According to story, physical examination and laboratory tests gastroenterologic malignity thought and gastroduodenal radiography with barium taken. In this radiography there was irregular filling defect at joint offundus and corpus.Because of this gastroscopy made and a 10x5x5 cm. phytobezoar has seen.During gastroscopy the phytobezoar cannot broken with polypectomi sneer and after surgical consultation gastrotomy decision taken. The bezoars defined as dryforeing bodies in stomach and seen very rare Eating inorganic materials and disorders of gastric motor functions increase frequency of formation of bezoars.Bezoars can be easily seen after routine daaily using of gastroscopy and strange because of their rarity.
