Tip II diabetli hastalarda kardiak otonom nöropati ve QT dispersiyonu

Özet
Çalışmamızın amacı, Kardiak otonom nöropatisi olan ve olmayan [KON(+) ve KON(-)] Tip II diabetli hastalarda kardiak aritmilerden ve ani ölümlerden sorumlu olduğu iddia edilen artmış QT dispersiyonu varlığını ve otonom nöropati semptomları ile diabetin diğer kronik komplikasyonlarını incelemektir. Çalışmamız Haseki Eğitim ve Araştırma Hastanesi 3. Dahiliye ve Endokrin Polikliniği'ne başvuran KON(+) 15 ve KON(-) 10, toplam 25 hasta ile Check-Up ünitemize başvuran 10 hastadan oluşan sağlıklı kontrol grubu üzerinde yapılmıştır. Manuel olarak QRS kompleksinin başlangıcından T dalgasının izoelektrik çizgiye döndüğü noktaya kadar olan mesafe, QT İntervali olarak alınmıştır. QT'nin en uzun olduğu değerden, en kısa olduğu değer çıkarılarak QT dispersiyonu (QTd) bulunmuş ve kalp hızının yanıltıcı etkisini gidermek için Bazzet formülü uygulanarak Corrected QT (QTc) elde edilmiştir. Çalışmamızda, ortalama QTc, QTd ve QTcd değerlerinin KON(+) grupta, hem KON (-) gruptan hem de sağlıklı kontrol grubundan anlamlı olarak yüksek bulunması, QTc veQTd artmasının otonom nöropatiyle ilişkili olduğunu göstermektedir.
Abstract
Our study's aim is to analyse the elevated QT dispersion which is said to be responsible for cardiac arythmias and sudden death in Type II diabetic patients who have cardiac autonom neuropathy or not [(KON(+) or KOR(-)] and autonom neuropathy symptoms and diabetic other chronical complications. Our study included KON(+) 15 and KON(-)10, all 25 patients who applied to the III. İnternal medicine department of Haseki Hospital and 10 healthy control group who were seen in our check-Up unit. Manually, the distance from the beginning of QRS complex till the point when T wave turned to the isoelectrical line is taken as QT intervale. The least value of QT is substracted from the highest value of QT to find QT dispersion(QTd) and by using Bazzet formula to correct the falsing effect of heart rate corrected QT(QTc) is gained. In our study finding the mean QTc, QTd and QTcd values in KON(+) group significant higher than KON(-) group and healthy control group shows that QTc and QTd elevation is associated with autonom neuropathy.
