Gastrik karsinomların preoperatif hidro bilgisayarlı tomografi (Hidro BT) ile değerlendirilmesi

Özet
Amaç: Çalışmamızın amacı endoskopik olarak gastrik malignite tespit edilmiş olan hastaların evrelendirilmesinde Hidro Bilgisayarlı Tomografi (Hidro BT) yönteminin kullanılabilirliği ve değerini araştırmaktır. Materyal ve Metod: Tümü endoskopik alarak tespit edilmiş ve patolojik olarak kanıtlanmış 14 erkek 8 kadın, 22 gastrik kanser olgusuyla çalışıldı. Radyolojik bulgularla operasyon sonuçları karşılaştırıldı. Hastalara mide distansiyonu için çekimden hemen önce optimal miktarda su içirildi. 100 cc nonionik kontrast madde intravenöz bolus tarzda uygulandı. Enjeksiyon bitiminden hemen sonra tomografik kesitler alındı. Bulgular: Olguların hepsi ileri evre idi. 16 olgu opere oldu. TNM evrelendirmesine göre tümörün duvar invazyonu için doğruluk oranı %87 (16 olgudan 14 ü), bölgesel lenf nodu tutulumu için doğruluk oranı %68 (16 olgudan 11'i) metastaz için doğruluk oranı %50 (16 olguda 8 olgu) olarak görüldü. Sonuç: Elde ettiğimiz sonuçlar ve görüntülerin kalitesi Hidro BT yönteminin gastrik malignite evrelendirilmesinde yararlı bir yöntem olduğunu ortaya koymaktadır.
Abstract
Purpose: The purpose of this study was to evaluate the utility of Hydro CT in the preoperative staging of endoscopically identified gastric carcinomas. Material and Method: We performed Hydro CT in 22 patients (14 male 8 female). All the cases were endoscopically identified and pathologically proved. Radiologic findings were correlated with surgical results. The patients drank optimal amount of water for the proper stomach distention just before the CT examination. We administered IV bolus 100 cc nonionic contrast agent. The scanning began just after the finishing of injection. Results: All the cases were advanced cancers. 16 of them were operated. According to TNM staging, the accuracy for the depth of tumor invasion is 87% (14 of 16 cases). For the regional lymph node spread, the accuracy is 68%. For distant metastasis, accuracy is 50%. (8 of 16 cases). Conclusion: The results we got and the quality of the images show that Hydro CT is valuable in staging gastric cancers.
