Özet
Lomber interbody füzyon, hareketli omurlar arasında gelişen siyataljili veya siyataljisiz kronik bel ağrılı hastalar için geliştirilmiş bir cerrahi uygulamadır. Dejeneratif disk hastalığı bel ağrısı ile birlikte radikulopati yapabilirler ve bu olgular simple diskektomiden fayda görmezler. Amaç hasta olan diski devre dışı bırakmak ve bu bölgenin füzyonunu sağlamaktır. Ağrının sebebi mekanik natürlüdür ve segmental mobiliteye bağlıdır. Hasta seçimi Food and Drug Administration tarafından belirlenmiştir. 1) Siyataljili veya siyataljisiz intraktibıl ve rahatsız edici bel ağrısı 2) Ağrılı dejeneratif disk hastalığı 3) Daha önce işlemin uygulanmamış olması 4) Grade 1 bel kayması 5) Komşu vertebralarda dejeneratif hadisenin olmaması. İnfeksiyon varlığı veya 12 mm den yüksek disk mesafesinin varlığı kontrendikasyon teşkil eder. Bu olguların cerrahiye almadan önce 1 yıl süre ile konservatif yöntemle izlenmesi gerekir. Cerrahi teknik vurgulanmadan konu olgularımızla birlikte literatür eşliğinde incelenmiştir.
Abstract
The interbody fusion was developed to deal with disabling low back pain with or without sciatica as a result of abnormalites of the motion segment. Dejeneratif disc disease may cause radiculopaty with leg pain accompanied by low back pain. Simple discectomy may not relieve pain. The purpose of this procedure is take out the disc material and achive a good fusion in the disc space. The cause of this pain is thought to be mechanical in nature and related to increased segmental mobility. Patient selection criteria used were described in the original Food and Drug Administration study: 1) severe disabling and intractable low back pain with or without sciatica 2) dejenerative painful disc disease 3) no prior interbody fusion at the designated level 4) spondylolisthesis Grade 1 5) An absence of degeneration at the adjacent segment. Contraindications include a disc space higher than 12 mm or the presence of disc space infection. All candidates for this procedure must have been symptomatic for more than 1 year and refractory to conservative care. The new literature has been reviewed in connection with our two cases. The surgical procedure is considered beyond the purpose of this puplication.
