Özet
Histerektominin geç komplikasyonlarından biri olan vajinal kaf prolapsusu, hastanın yaşam kalitesini bozan bir faktör olarak %0.2-%4.5 insidansa sahiptir. Abdominal sakral kolpopeksi operasyonu %90 başarı oranıyla vajinal kaf prolapsusunu en kuvvetli ve en uzun süre düzelten operasyondur. Ancak maalesef bu operasyonda posterior uretrovesikal açının değişmesi veya operasyon sonrası belirgin hale gelen fark edilmemiş üretral sfinkter yetmezliği sebebiyle %5 civarında postoperatif gelişen üriner inkontinansa neden olma riski vardır. Bu olgunun sunulmasındaki amaç vajinal kaf prolapsusu tedavisi sonrasında intrensek sfinkter yetmezliğe bağlı üriner inkontinans gelişebileceğini vurgulamak ve primer operasyonla aynı seansta subüretral sling operasyonu planlamak için dikkatli bir preoperatif değerlendirme gerekliliğini vurgulamaktır.
Abstract
Being one of the late complications of hysterectomy, vaginal vault eversion is a factor disturbing the quality of a patient's life and has an incidence of 0.2%-4.5%.Abdominal sacrocolpopexy operation with its 90% success rate is the most strong operation type and has the longest period of time in maintanance of correcting of vaginal vault eversion. However unfortunately, this operation has about 5% risk of postoperative urinary incontinence risk because of the changing of the posterior urethrovesical angle or undetected deficiency of urethral sphincter which becomes obvious postoperatively. The goal representing this case is to declare that urinary incontinence can occur because of intrinsic sphincter insufficiency following treatment of vaginal vault eversion and to focus careful preoperative evaluation should be done to plan a suburethral sling operation in the same sitting in the primary surgery
