Previous poor obstetrical history: Is it associated with abnormal umbilical artery doppler findings in a subsequent pregnancy

Özet

Bu çalışmanın amacı, kötü obstetrik öyküsü olan gebeliklerde, gebeliğin iki ayrı döneminde umbilikal arterlerde saptanan Doppler akım hızlarının anormal olup olmadığını değerlendirmektir. Kötü obstetrik öykü daha önceki gebeliklerde, tekrarlayan abortus, perinatal ölüm ve fetal anomalileri kapsayan en az iki gebelik kaybı olarak tanımlandı. Kırkaltı normal.(kontrol), 40 kötü obstetrik öykülü gebede umbilikal arter pulsatilite indeksi (Pl), rezistivite indeksi (Rl) ve sistol/diastol oranı (S/D) iki donem halinde incelendi. Gebelik haftaları dönem l'de 20-31, dönem İlde 32-42 arasında değişmekteydi. Olguların klinik, laboratuar ve biyokimyasal bulguları ile gebelik sonuçları değerlendirildi. Her iki gebelik . döneminde de umbilikal arter Doppler ortalama değerleri, kötü obstetrik öykülü grupta, kontrol grubuna göre istatistiksel olarak anlamlı yükseklik gösterdi. İzole anormal bulgular ise özellikle 31. gebelik haftasından sonra gözlendi. Kötü obstetrik öyküsü olanlar mevcut gebelikleri sırasında dikkatli izlenmelidirler. Bu gebeliklerdeki fetuslar, gebeliğin değişik dönemlerinde ve özellikle geç gebelik döneminde Doppler v ultraşonögrafi ile saptanabilen anormal fetoplasentai dolaşım özelliğine adaydırlar.

Abstract

OBJECTİVE: The aim of this study was to assess if pregnancies with previous poor obstetrical history are associated with or without abnormal Doppler flow velocity findings in umbilical arteries during two different periods of pregnancy. STUDY DESIGN: Poor obstetrical history is defined by at least two pregnancy losses in the previous pregnancies including recurrent abortions, perinatal deaths or fetal birth defects. Umbilical artery pulsatility index (PI), resistance index (Rl) and systole to diastole ratio (S/D) of 46 pregnancies with normal obstetrical history (controls) and 40 pregnancies-'with poor obstetrical history, were evaluated by color Doppler ultrasonography into two periods. Gestational'weeks ranged between 20-31 in period I and between 32-42 in period II. Clinical, laboratory and biochemical findings and current obstetric outcome of the cases were evaluated. RESULTS: Statistically significant high Doppler values on umbilical artery mean indices and ratios were determined in the poor obstetrical history group on both gestational periods, when compared with the control group. Isolated abnormal findings were seen especially after the 31 st gestational week. CONCLUSION: Pregnancies with poor obstetrical history have to be observed with care in a subsequent pregnancy. Their fetuses are candidates of abnormal characteristics of feto-placental circulation that are detectable by Doppler ultrasonography during different gestational periods and especially in late gestation.
