Effects of tamoxifen on endometrium in postmenopausal breast cancer patients

Özet

Jinekolojik olarak asemptomatik 47 postmenopozal meme kanserli hastada, tamoksifen'in endometriyum üzerine etkileri araştırıldı. Hastalardan 26'sı en az 6 ay süre ile tamoksifen kullanmaktaydı. Kontrol grubunu oluşturan 21 hasta ise herhangi bir hormonal tedavi almamaktaydı. Tamoksifen alan hastalarda ortalama endometriyal kalınlık 7.9+5.8 mm, kontrol grubunda 3.7+2.0 mm olarak bulundu; aradaki fark istatistiksel olarak anlamlı idi (p<0.05). Endometriyal örneklemelerde, tamoksifen alan hastaların 8 inde (%30.7) patolojik değişiklikler saptanrken, kontrol grubunda 1 hastada (%4.7) patolojik bulgu mevcuttu; aradaki fark istatistiksel olarak anlamlı idi. (p<0.05). Bu bulgularla, tamoksifen kullanan postmenopozal hastalarn peryodik endometriyal biyopsilerle takip edilmeleri gerektiği sonucuna varıldı.

Abstract

OBJECTIVE: To assess the effects of tamoxifen on endometrium in postmenopausal breast cancer patients. STUDY DESIGN: Forty-seven gynecologically asymptomatic postmenopausal breast cancer patients were studied. Twenty-six patients had been treated with tamoxifen for at least 6 months and 21 had not received any hormonal treatment. The two groups were similar in age, parity, age at menopause and body mass index. RESULTS: The mean ultrasonographic endometrial thicknesses in tamoxifen-treated and nontreated patients were 7.9±5.8 mm and 3.7±2.0 mm respectively; the difference was statistically significant (p<0.05). The frequency of endometrial pathologic changed in the endometrial samplings was significantly higher (30.7%) among the tamoxifen-treated patients, compared with nontreated patients (4.7%) (p<0.05). In tamoxifen group, pathologic endometrial findings were observed in 8 (30.7%) patients. Of these women 5 (19.2%) had an endometrial polyp, 2 (7.7%) had endometrial cystic hyperplasia and 1 (3.8%) had endometrial cancer. In control group, pathologic endometrial change was diagnosed in 1 (4.7%) patient who had endometrial cystic hyperplasia. CONCLUSION: Our findings indicate a significant correlation between tamoxifen treatment and pathologic endometrial changes in postmenopausal breast cancer patients. We therefore recommend that postmenopausal patients receiving tamoxifen have periodic endometrial samplings.
