Interleukin 1-beta levels in maternal serum and amniotic fluid before and after onset of active labor in normal pregnancies

Özet

Fetus ya da anne tarafından yapılan İL 1-P'nın doğum ağrılarının başlamasında sinyal görevi gördüğü ileri sürülmüştür. Çalışmamızın amacı normal gebelerde doğum ağrıları başlamadan önceki ve aktif doğum eylemi sırasındaki maternal serum ve amniotik sıvı İL 1-P düzeylerini değerlendirmektedir. Bu amaçla 37-40 haftalık gebelerde doğum ağrıları başlamadan önce maternal serum ve. amnion sıvı örnekleri alındı. Aynı hastalarda doğum için başvurduklarında aktif doğum eylemi sırasında ikinci kari ve amnion sıvı örnekleri alındı. Doğum eylemi öncesinde ve aktif doğum ağrıları sırasında alınan maternal serum örneklerinde İL 1-p düzeyleri yönünden anlamlı bir fark bulunamadı. Doğum eylemi öncesinde alınan amnion sıvısında İL 1-p düzeyi 15,73 (+12,77) pg/ml iken aktif doğum ağrıları sırasında alınan amnion sıvısında'İL 1-0-düzeyleri 227,20 (±55,54) pg/ml olarak bulundu. Aradaki bu fark istatistiksel olarak anlamlı idi (p<0.0Ö5). Bütün bu bulgular ışığında aktif doğum eylemi sırasında.amnjoh sıvısında.İL 1-p düzeylerinin artmasının korio-desidıial bileşkedeki hücrelerin inflamasyonu ya da hasarına bağlı olabileceği düşünüldü.

Abstract

OBJECTIVES: It has been postulated that Interleukin 1-p (IL-1p). produced by the fetus or mother could serve as a signal for initation of labor. Our objective was to evaluate the IL 1-p levels in maternal serum and amnioitic fluid before and after the onset of active labor in normal pregnancies. STUDY DESIGN: A cohort study design was used to compare the maternal serum and amniotic fluid IL 1-P levels befroe and after the onset of active labor in normal pregnancies. Paired (n=10) maternal venous blood and amniotic fluid samples were obtained from women not in labor (at 37-40 Wks) and second samples from the same group were obtained after the onset of active labor. RESULTS: No significant difference was found betweeti the maternal serum IL 1-p levels either before and after the onset of active labor. Amniotic fluid IL 1-:p concentration was found to be 15,73±12.77 pg/ml and 227,20±55,54 pg/ml before and during active labor respectively. The difference was singificant at p<0.005. No significant correlation was found between the maternal serum and the amniotic fluid IL 1-p levels either before or after the onset of active labor. CONCLUSION: The onset of active labor is associated with a significant increase in the concentrations of IL 1-p in the amniotic fluid compartments but not in maternal serum. All these findings suggest that the increase of the IL 1-p level of amniotic fluid after the onset of active labor may result from inflammation or injury of the cells at the chorio-decidual junction and IL 1 -p might be just a mediator or a marker of inflammation that accumulates in amniotic fluid as a normal sequel of labor.
