The treatment of chronic pelvic pain by laparoscopic uterine nerve ablation

Sebebi bilinmeyen pelvik ağrı tedavisi daima önemli bir problem olmuştur. Kronik pelvik ağrı Amerika Birleşik Devletleri'ndeki histerektomilerin %12'sinin ve laparoskopilerin ise % 40'nın sebebidir. Tıbbi tedaviye yanıt vermeyen, ultrasonografide organik patolojisi saptanmayan dismenore, disparenü gibi şikayetleri olan hastalara LUNA uyguladık. 1992 yılından itibaren 110 olguya uyguladık. Olgulardan 49'u operasyon sırasında organik patolojileri saptandığı için çalışma dışında tutuldular. Hastaların ağrı skorları lineer analog skala ile ölçüldü. Postoperatif 1., 2. ve 3. Yıllarda ağrı skorlarında % 61.5, % 52.4 ve % 38.7' lik bir düşüş saptadık. Kronik pelvik ağrıda diagnostik ve operatif laparoskopinin değerini belirlemek istedik.
Abstract
OBJECTIVE: The treatment of pelvic pain of unknown reason has always been an important problem. Chronic pelvic pain(CPP) is reported as the cause of 40% of laparoscopies and 12% of hysterectomies performed in USA. Laparoscopic uterine nerve ablation con be used for treatment. STUDY DESIGN: We applied laparoscopic uterine nerve ablation (LUNA) to the patients who had complaints such as dysmenorrhea, dyspareunia, pelvic pain who had no organic pathology ascertained by ultrasonography and did not respond to any medical treatments. RESULTS: Since 1992, we performed LUNA for the 110 patients. We excluded 49 patients because of their organic lesions detected in the operations. Pain score of the patients were measured by linear analog scala. We found out that pain scores decreased of 61.5%, 52.4%, 38.7% at the postoperative 1st, 2 and 3rd years respectively. CONCLUSION: We discussed the value of diagnostic and operative laparoscopy in CPP.
