Effect of body mass index on single ultrasonic screening of endometrial thickness in asymptomatic postmenopausal women

Özet

Bu çalışma postmenopozal dönemde endometrium patolojilerinin taranmasında sık kullanılan 5mm kestirim değerinin tanısal değerini ortaya koymak ve vücut kitle indexinin (BMI) taramaya etkisini araştırmak için planlanmıştır. Bu amaçla 142 asemptomatik postmenopozal hasta vaginal ultrasonografi ile taranmış, endometriyum kalınlığı >5 mm olan olgulara küretaj yapılmıştır. Lojistik regresyon analizi sonucunda hastaların menopozda oldukları süre arttıkça >5 mm endometrium varlığının azaldığı bulundu. BMI>30 olan olgular obez kabul edildiğinde obez hastaların endometriumlarının ortalama kalınlığının obez olmayanlardan istatistiksel olarak anlamlı farklılığının bulunmadığı görüldü. Ayrıca BMI' deki artışın >5 mm endometrium varlığına etkisi olmadığı bulundu. Küretaj yapılan 42 hastanın hiçbirinde endometrium kanserine rastlanmadı. Bu bulguların ışığında >5mm endömetriyumun asemptomatik postmenopozal kadınlarda sık görüldüğü, frekansının menopozda kalınan süreyle ters orantılı olduğu ancak BMl'inden etkilenmediği sonucuna varıldı.

Abstract

OBJECTIVE: To determine the affect of Body Mass Index (BMI) on single transvaginal ultrasonographic screening of endometrial thickness for endometrial cancer in asymptomatic postmenopausal women not having hormone replacement therapy. STUDY DESIGN: One hundred and forty two asymptomatic postmenopausal women with a minimum twelve months since last menstrual period were included in the study. Age, years since menopause and BMI were recorded for each subject and curettage was performed in patients with a 5 mm or thicker endometrium. The affect of menopausal period and BMI on the presence of a 5 mm or thicker endometrium was assessed with logistic regression analysis. RESULTS: The mean age of our menopausal population was found to be 51,7 years ± 4,7. The weight of the women was between 45-110 kg (mean=71,1±12,04). Patients were found to be 157 cm ± 5,9 tall. Mean menopausal period was 21,1 months ± 8 (range=12-60) and transvaginal ultrasonographic thickness of endometrium was ranging between 1-12 mm (mean=3,7 ± 2,3). Logistic regression analysis revealed the fact that increases in menopausal period cause a significantly lower percentage of 5 mm or thicker endometrium (OR=0,8, CI=0,8-0,9). Increases in BMI were found not to change the percentages of 5 mm or thicker endometrium (OR=0,8, Cl=0,4-1,5, p=0,5). The mean endometrial thickness of obese women with a BMI>30 was not significantly different from women with a BMI <30 (m=4,06±2,7, m=3,4±1,9, p=0,1 respectively). There was no statistically significant difference in the menopausal periods of the two groups (m=20,7±7,6, m=21,4±8,2, p=0,5 respectively). Forty-six patients (33%) were found to have a 5 mm or thicker endometrium and pathologic examination could not detect any case of endometrial cancer. CONCLUSION: Prevalence of 5 mm or thicker endometrium was high in asymptomatic women. The prevalence was inversely correlated with menopausal period and was not affected by BMI.
