Outcome of twin pregnancies with single intrauterine fetal demise

İkiz gebelikte, tek fetusun intrauterin ölümüne sık rastlanılmamaktadır. Bu çalışma, intrauterin tek ölü fetuslu ikiz gebelik olgularındaki perinatal ve maternal sonuçları belirlemek amacıyla yapılmıştır. 20 haftadan büyük gebelik olgularının dahil edildiği retrospektif bir çalışmadır. Mart 1996 ve Ocak 1998 tarihleri arasında 40.089 canlı doğum olmuştur, bunların 429'u ikiz gebelik olarak belirlenmiştir (%1.07). İkiz gebeliklerin 9'unda (%2.09) intrauterin tek ölü fetus saptanmıştır, bu olgulardaki 4 yenidoğan, yoğun bakım ünitesine alınmış, 1 yeni doğan ciddi respratuar distress sendromu nedeniyle kaybedilmiştir. Konjenital anomali gözlenmemiştir. Üç hastada hafif preeklampsi gelişmiştir ve konservatif tedavi sırasında hiçbir olguda tüketim koagülopatisi izlenmemiştir. Konservatif tedavi, maternal ve fetal durumun yakın takibiyle birlikte uygulanıldığında, intrauterin tek ölü fetuslu ikiz gebelik olgularında savunulabilecek bir tedavi şeklidir.
Abstract
OBJECTIVE: Intrauterine fetal demise of one fetus in a twin gestation is an uncommon occurrence. Our purpose was to examine the perinatal and maternal outcomes in twin pregnancies with a single fetal demise. STUDY DESIGN: This was a retrospective study and included greater than 20 weeks' gestations. RESULTS: Between March 1996 and January 1998, there were 40.089 live deliveries, of which 429 were documented as twin pregnancies(1.07%). Of these twin pregnancies 9(2.09%) were complicated by a single intrauterine death. Four neonates were admitted to special care units. One infant died from cause of severe respiratory distress syndrome. Congenital anomaly was not observed. Three patients had developed mild pregnancy induced hypertension. Consumptive coagulapathy was not occurred in these patients during conservative management. CONCLUSION: With a close observation of maternal and fetal conditiones, conservative management of a single fetal demise of twin pregnancy can be advocated
