Serum haptoglobin levels in severe preeclampsia, eclampsia and hellp syndrome

Özet

Bu çalışmada serbest hemoglobini bağlayan haptoglobinin, HELLP sendromunda hemolizin belirleyicisi olarak değerlendirilmesi amaçlandı. Serum haptoglobin seviyeleri üç grup hastada doğum öncesi ve doğumdan 5 gün sonra ölçüldü: HELLP (n=15), şiddetli preeklampsi veya eklampsi (n=26) ve kontrol grubu (n=21).' Antepartum ortalama haptoglobin seviyeleri bu üç grupta sırasıyla 37,44+1,37, 74,45+3,27, ve 182,16+13,29 mg/dl olup birbirlerinden anlamlı olarak farklıydı (p 0,05). Doğumdan sonra haptoglobin seviyelerinin HELLP ve şiddetli preeklampsi/eklampsi gruplarında yükseldiği (kontrol ile karşılaştırılınca p>0,05), fakat indirek bilirubin seviyelerinin HELLP grubunda halen anlamlı olarak yüksek olduğu görüldü. Haptoglobinin, hemolizin sensitif ve erken bir belirleyicisi olduğu sonucuna varıldı.

Abstract

OBJECTIVE: To asses haptoglobin, which binds the free hemoglobin, as a marker of hemolysis in HELLP syndrome. STUDY DESIGN: Serum levels of haptoglobin were determined before and 5 days after the delivery in 3 groups of patients: Severe preeclampsia or eclampsia (n=26), HELLP (n=15) and control (n=21). Mann-Whitney U, Wilcoxon Rank Sum W test and correlation analysis were used for statistical analyses. RESULTS: Antepartum mean haptoglobin levels were 37.44±1.37, 74.45+3.27 and 182.16+13.29 mg/dl in the HELLP, severe preeclampsia/eclampsia and control groups respectively, significantly different from each other (p<0.05). Indirect bilirubin levels were high in HELLP syndrome indicating the hemolysis (p<0.05), but not significantly different in severe preeclampsia/eclampsia and control groups. After the delivery, haptoglobin levels promptly increased in the HELLP and severe preeclampsia/ eclampsia groups (p>0.05 compared with control); but indirect bilirubin levels were still significantly high in the HELLP group. CONCLUSION: Haptoglobin is a sensitive and an early marker of hemolysis.
