Giant myoma and the myomatous erythrocytosis syndrome

Özet

Bu çalışmanın amacı dev subseröz myomlu olguda ortaya çıkan myomatöz eritrositoz sendromunun tartışılmasıdır. Laparatomi öncesinde flebotomi yolu ile dört ünite kan alınan olgudan 14.2 kilogramlık subseröz myom çıkarıldı. Hematolojik parametreler pre ve post-operatif dönemde izlendi. Pre-operatif dönemde % 53.3 olan hematokrit ve 17.5 g/dl olan hemoglobin düzeyleri dört ünite flebotomi sonrası sırasıyla % 48.6 ve 16.8 g/dl'ye düştü. Dev myom çıkarıldıktan 10 gün sonra ise hematokrit % 40.3 ve hemoglobin 14.3 g/dl düzeyine spontan olarak geriledi. Bubulgu bize batın içindeki dev myomun eritrositoz tablosuna neden olduğunu düşündürdü.

Abstract

The objective of this study is to discuss the myomatous erythrocytosis syndrome in a patient with giant subserous myoma. A huge subserous myoma was discovered in a case after the venesection of four units of blood. Hématologie parameters are followed up pre and post-operatively and six months later after the operation. After venesection of four units of blood, preoperative hematocrit value of 53.3 % and hemoglobin value of 17.5 g/dl had decreased down to 48.6 % and 16.8 g/dl levels, respectively. By the operative extraction of giant subserous myoma weighing 14.2 kg, the hematocrit and the hemoglobin values were both regressed to 40.3 % and 14.3 g/dl levels, respectively. These findings showed us that the giant subserous myoma was the cause of the myomatous erythrocytosis syndrome.
