Özet

Bu çalışmada depo medroksiprogesteron asetatı (DMPA) bir kontraseptif yöntem olarak seçen kadınların özelliklerini ve bu yöntemle ilgili deneyimlerini ortaya koymak amaçlandı. Mart 1995-Nisan 1997 arasında toplam 1292 DMPA enjeksiyonu yapılan 393 kadın hakkındaki klinik bilgiler retrospektif olarak incelendi. Çalışma kapsamındaki olguların ortalama yaşının 28.3n5.9 (18-48) olduğu bulundu. Ortalama parite sayısı 2.22n1.09 idi. Olgu başına ortalama 3.28 enjeksiyon uygulandığı bulundu. Ortalama takip süresinin 9.56 ay olduğu belirlendi. Olguların 248'inde ilaca bağlı değişik yan etkiler geliştiği saptanırken (%63.1) 145 olgu hiçbir yan etki tariflemedi (%36.9). Bildirilen yan etkilerin büyük bölümü menstruel kanama düzensizlikleri ile ilgiliydi (%56.5). Takip süresince 79 olguda (%20.1) kilo artışı olduğu belirlendi. Diğer yan etkilere göreceli olarak daha az oranda rastlandı; bunlar arasında en sık görülen şiddetli başağrısı idi (%3.8). Olguların %31'i yan etkiler nedeniyle yöntemi terkederken 153 olgu (%38.9) kontrasepsiyon yöntemini değiştirerek yöntemi bıraktı. On sekiz olgu (%4.6) gebelik arzusu ile yöntemi terketti. Çalışma kapsamındaki 100 olgu ise herhangi bir neden bildirmeksizin yöntemi bıraktı. DMPA ülkemizde yeni kullanıma giren bir kontraseptif yöntemdir. Bu çalışmada DMPA'ya bağlı yan etkiler nedeniyle olguların yüksek oranda yöntemi terkettiği gösterilmiştir.

Abstract

OBJECTIVE: To determine the characteristics and experience of Turkish women treated with depot medroxyprogesterone acetate (DMPA) as a contraceptive method at a teaching hospital in Ankara. DESIGN: A detailed retrospective review of clinical data on 393 women treated with a total of 1292 injections of DMPA during the period March 1995-April 1997. RESULTS: The mean age of the subjects included in the study was found to be 28.3±5.9 (range: 18-48). The mean parity of the study group was found to be 2.22±1.09. There was a low acceptability, with a mean of 3.28 injections per subject and a mean overall follow-up of 9.56 months. Only 145 subjects reported no side effects (36.9%) whereas 248 of them complained of various kinds of side effects (63.1%). Majority of the reported side effects belonged to menstrual bleeding disturbances (56.5%).Weight gain was observed in 79 patients (20.1%) in the follow-up period. Other side effects were relatively infrequent, the most common being serious headache (3.8%). Side effects led to 31% of the discontinuations whereas 153 patients left the method with change of contraception or no further need (38.9%).There was desired pregnancy in 18 subjects (4.6%) and 100 patients decided to discontinue without reporting any reason (25.4%). CONCLUSION: DMPA is a newly introduced contraceptive method in Turkey. This study revealed high rates of drop-outs due to DMPA related side effects in our metropolitan population.
