The effect of hormone substition therapy on plasma endothelin-1 level, lipid profile, and body weight

Postmenapozal kadınlardaki hormon tedavisinde vasküler epitel endotelin-1 gibi vazoaktif maddeler üreterek, östrojenin kardiyoprotektif etkisini sağlamada önemli bir rol oynayabilir. Biz çalışmamızda, 25 postmenapozal kadında 24 hafta süren, estradiol valerate 2 mg + ciproterone acetate 1 mg/gün tedavisinden önce ve sonra endotelin-1 ve plazma lipidlerini ölçtük ve vücut kitle yoğunluğunu hesapladık. Ortalama endotelin-1 düzeyi tedaviden önce 5.2±1.7 pmol/l iken, tedaviden sonra 4.9±1.6 pmol/l bulundu. Anlamlı bir değişiklik olmadı (p=0.324). Total kolesterol düzeyi 243.3±37.4 mg/dl den 221.3±28.8 e (p=0.0003) ve trigliserid düzeyi 154±58 mg/dl den 124.3±42.5 a (p=0.001) anlamlı olarak düştü. LDL-kolesterol düzeyindeki değişiklik de anlamlı bulunmadı. HDL-kolesterol düzeyi anlamlı olarak 43.2±8.0 mg/dl den 49.8±8.8 e yükseldi (p=0.026). Ortalama vücut kitle yoğunluğu da anlamlı olarak 28.4±2.7 den 29.9±2.9 e (p=0.001) yükseldi. Biz, östrojenin lipid profili üzerine olumlu etkisinin bulunduğu ve kilo artışına hafif bir katkıda bulunduğu ancak, endotel üzerine olan etkisinin değerlendirilebilmesi için daha fazla araştırmaya gereksinim olduğu sonucuna vardık.
Abstract
OBJECTIVES: The aim of the study was to assess the role of endothelin-1 and plasma lipids in the cardioprotective effects of estrogen therapy on vascular endothelium in postmenopausal women. METHODS: We measured endothelin-1 levels and plasma lipids before and after the daily use of estradiol valerate 2mg + ciproterone acetate 1 mg for 24 weeks in 25 postmenopausal women. RESULTS: The mean endothelin level was 5.2±1.7 pmol/l before the therapy, and 4.9±1.6 pmol/1 after the hormone replacement. The difference was not significant (p = 0.324). The mean levels of total cholesterol decreased significantly (from 243.3±37.4 mg/dl to 221.3±mg/dl), and triglyceride (154±58 mg/dl to 124.3±42.5 mg/dl) after the therapy. The change in the LDL-cholesterol levels was not significant. The mean HDL-cholesterol level was significantly increased from 43.2±8.0 mg/dl to 49.8±8.0 mg/dl. The mean calculated body mass index (BMI) was significantly increased from 28.4±2.7 to 29.9±2.9 after the therapy. CONCLUSIONS: We concluded that, hormone replacement therapy has favorable effects on lipids and lipoproteins, and may cause a slight weight gain, but further studies are needed to evaluate the endothelial effects of estrogens.
