Maternal serum triple-marker levels for Down syndrome screening in spontaneously conceived and in IVF pregnancies

Özet
Bu çalışmanın amacı spontan tek gebeliklerle invitro fertilizasyon sonucu elde edilmiş gebeliklerde maternal üçlü test sonuçlarının karşılaştırılmasıdır. Bu amaçla 51 spontan gebelikle, IVF ile elde edilmiş 30 tek gebelikte maternal serum alfafetoprotein, human koryonik gonadotropin ve estriol değerleri incelenmiş ve "multiple of median" (MOM) olarak karşılaştırmıştır. IVF gebeliklerde HCG, AFP ve ue3 değerleri sırayla 1.237, 1.149 ve 1.629 olarak bulunurken, kontrol grubunda bu değerler yine sırayla 1.621, 1.106 ve 1.941 olarak bulunmuştur. Yaş ortalaması IVF grubu için 31.2, kontrol grubunda ise 28.8 olarak hesaplanmıştır. HCG, AFP ve ue3 değerleri arasında çalışma grubu ve kontrol grubu arasında istatistiki bir farklılık bulunmamaktadır. Bu bulgu daha önce yayınlanmış bazı çalışmalara aykırı düşmekle beraber IVF ile elde edilmiş tek gebeliklerde üçlü test sonuçlarının bir tarama testi olarak kullanılabileceğini göstermektedir. Ancak son sözü söylemek için daha geniş hasta gruplarında çalışmaya ihtiyaç vardır.
Abstract
OBJECTIVE: The aim of this study is to document and to compare the maternal serum analyte levels in spontaneously conceived pregnancies and in pregnancies occurred as result of in vitro fertilization. STUDY DESIGN: Maternal serum human chorionic gonadotropin, alpha fetoprotein and estriol levels were determined in 30 singleton IVF pregnancies and 51 spontaneously conceived pregnancies which served as control during one year period. The levels were compared as multiple of median (MOM). RESULTS: In IVF pregnancies mean HCG, AFP and ue3 levels were 1.237, 1.149 and 1.629 respectively whereas in control group these figures were 1.621, 1.106 and 1.941. The mean age was 31.2 for IVF group and 28.8 for controls. The difference of mean HCG, AFP and ue3 levels were not statistically significant between the groups CONCLUSION: The hCG, AFP and estriol levels were not significantly different in both groups. Although this finding is contradictory to earlier reports, triple test results can be used as a screening test in pregnancies occurred as result of IVF. However, it is early to state a final conclusion in this issue and controlled studies in larger samples are needed.
