Cardiac function in fetuses of gestational diabetic mothers

Özet
Amaç: iyi glukoz kontrollü gestasyonel diyabetik annelerin fetüslerinde kardiak fonksiyonu değerlendirmek ve bunu normal annelerin fetüsleri ile kıyaslamak Gereç ve Yöntem:Üçüncü trimesterde iyi kontrollü gestasyonel diyabetik 28 gebenin fetüslerinde M-mode ve Doppler ekokardiyografik çalışmalar yapıldı. Ölçülen parametreler şunlardı: 1) End diyastol ve pik sistol sırasında interventriküler septum, sağ ventrikül ve sol ventrikül duvar kalınlıkları. 2) Aynı seviyede end diyastol ve pik sistol sırasında sağ ve sol ventrikül iç çapları. 3) Erken ventriküler dolum sırasında (E dalgası) ve atrial sistol sırasındaki (A dalgası) mitral ve triküspid pik velosite'leri, E/A oranı ve mitral ve triküspid akım dalga formu time average velosite' leri. 4) Umbilikal arter akım dalga formu indeksleri. Bulgular: Ortalama gestasyonel yaş ve ortalama fetal biyometrik parametreler arasında guruplar arasında fark saptanmadı. Ortalama interventriküler septal kalınlık diyabetik annelerin fetüslerinde anlamlı olarak daha fazla idi. Diğer parametreler guruplar arasında farklılık göstermiyordu. Sonuç: İyi kontrol edilmiş gestasyonel diyabet interventriküler septal kalınlık dışında diğer fetal kardiak fonksiyonları anlamlı olarak etkilemez.
Abstract
OBJECTIVE: The purpose of this study was to determine the cardiac function in fetuses of mothers with strictly controlled gestational diabetes, and to compare these findings with those of normal fetuses. STUDY DESIGN: M- mode and Doppler echocardiographic studies were performed in 28 fetuses of well controlled gestational diabetic mothers and in 20 control fetuses at third trimester gestation. The measured variables were: 1) Wall thickness during end diastole and peak systole at the levels of the interventricular septum, left ventricle, and right ventricle. 2) Right and left ventricular inner dimensions at the same levels during peak systole and end diastole. 3) Tricuspid and mitral peak velocities during early ventricular filling (E wave), and atrial systole (A wave), E/A ratios, and time average velocities calculated on the mitral and tricuspid velocity waveforms. 4) Umbilical artery blood velocity waveform indices. RESULTS: There was no difference in the mean gestational ages, and the mean fetal biometric parameters between the groups. The mean interventricular septal thickness was significantly higher in fetuses of diabetic mothers. No significant differences were found among the two groups in means of other parameters. CONCLUSION: Well controlled gestational diabetes does not significantly affect the fetal cardiac functions except the interventricular septal thickness.
