A clomiphene citrate and tamoxifen combination therapy in ovulation induction

Ovulasyon İndüksiyonunda klomifen sitrat (CC) ve tamoksifen (TMX) kombine tedavisinin etkisi, prospektif çapraz bir çalışmada, tek başına klomifen tedavisi ile karşılaştırıldı. Kırkdört normoprolaktinemik anovulatuar kadın, rasgele grup A ve grup B olmak üzere iki gruba ayrıldı. Yalnız CC tedavi protokolünde;44 kadının 22'si (grup A, n:22), ard arda gelen üç tedavi siklusu boyunca, siklusun 5. ve 9. günleri arasında yalnız 100 mg/gün CC ile tedavi edildi. Bu hastalara, daha sonra gelen üç menstruel siklus boyunca, CC/TMX kombine tedavisi için siklusun 5. ve 9. günleri arasında 50 mg/gün CC ve 20 mg/gün TMX verildi. Geri kalan 22 kadına (grup B, n:22), yukarıdaki tedavinin tersine, aynı sürede ve aynı dozda, önce CC/TMX kombinasyon tedavisi, takiben yalnız CC tedavisi uygulandı. Ovulasyon olduğu, ultrasound kriterlerine ve mid-luteal progesteron seviyelerine bakılarak belirlendi. Yüzyirmi CC/TMX siklusu çalışıldı ve 84 siklusta (%70) ovulasyon oldu; bunların 6'ı (% 7.1) gebe kaldı. Yalnız CC alan grupta, toplam 115 siklusun 48'i (%41.7) ovulatuardı; bunların 2'i (% 4.1) gebe kaldı. Bu çalışma ile, CC/TMX kombinasyon tedavisinin, anovulasyonlu kadınlarda, ovulasyon İndüksiyonunda tek başına CC tedavisine göre daha etkili olduğu kanıtlandı.
Abstract
OBJECTIVE: To compare the effectiveness of clomiphene citrate (CC) and tamoxifen (TMX) combination therapy in inducing ovulation with a clomiphene alone therapy in a prospective crossover study. STUDY DESIGN: CC alone and CC/TMX combination treatment were employed in the randomized cross-over study. Forty -four normoprolactinemic anovulatory randomly divided into groups A and B. For the CC alone therapy, 22 of the 44 women (Group A, n:22) were treated with CC 100 mg alone during cycle days 5 to 9 for the initial three consecutive menstrual cycles . For the combination therapy, these patients received CC 50 mg and TMX 20 mg daily from day 5 to 9 for subsequent three consecutive menstrual cycles. To the remaining 22 women (Group B, n:22) was applied similarly the inverse sequence of treatments. Ovulation was determined by ultrasound criteria, and 9 rise in midluteal progesterone >7 ng/ml. RESULTS: In the CC alone group, 48 of 115 cycles (41.7%) were ovulatory; of these, 2 (4.5%) was conceived. In this treatment group, there was 1 set of twins and other was singleton pregnancy. One hundred-twenty CC/TMX cycles were studied. Eighty-four of 120 (70%) ovulated. Six of those (13.6%) conceived. In the CC /TMX cycles, all pregnancies were single and normal. The combination of CC /TMX was more effective in ovulation induction than CC alone therapy. CONCLUSION: In this paper, we conclude that the combination of CC/TMX was providing high efficacy in inducing ovulation in women with anovulation.
