The importance of Doppler ultrasonography, ultrasonographic morphologic scores and Ca-125 in differential diagnosis of pelvic masses

Özet
Amaç: Bizim çalışmamızın amacı Doppler ultrasonografi yoluyla adneksial kitlelerdeki kan akım değişikliklerinin benign-malign ayırımındaki yerini saptamak; ultrasonografik morfolojik skorlama sistemi, tümör belirteçleri ve histopatolojik sonuçları ile karşılaştırmaktır. Materyal-Metod: Pelvik kitle saptanan ve operasyon planlanan 59 hastada trombosit sayısı, Ca125 değeri, ultrasonografik morfolojik skorlama sistemi puanı, Doppler S/D, Rl ve Pl değerlerinin preoperatif adneksial malignite tanısındaki yerini değerlendirmede; sensitivite, spesifisite, pozitif ve negatif prediktif değerler hesaplandı. Tanısal yöntemlerle elde edilen pozitif ve negatif sonuçlar kitlenin histopatolojik sonuçları ile karşılaştırıldı. Bulgular: 59 olgunun 41'i benign, 18'i malign özellikte idi. Ultrasonografik skorlama sistemi puanı (>5 malign) ve Ca125 değerleri (cut-off > 35 IU/ml) malign kitlelerde istatiksel olarak anlamlı bulundu. Malign olgularda Doppler Rl (cut-off<0.6), S/D (cut-off<2.3) ve Pl (cut-off<1.0) değerleri benign olgulara nazaran istatiksel olarak anlamlı bir şekilde düşük bulundu. Sonuç: Reproduktif dönemde adneksial kitlelerin malignite açısından değerlendirilmesinde Doppler , ultrasonografi bulguları içinde Pl yüksek sensitivite (%100) ve negatif prediktif değer (%100) ile en kullanışlı parametre olacak görülmektedir.' Ca125, Doppler Pl, Rl ve S/D oranı lie ultrasonografik morfolojik skorlama sistemlerinin malign neoplazilerin ayırıcı tanısında birbirine yakın özgünlüğe sahip olduklarını saptadık. Tüm parametreler birlikte değerlendirildiğinde sensitivite ve negatif prediktif değerin yüksek olduğunu tespit ettik. Bu da bize yanlış negatiflik sonuç oranının azalmasına karşın, yalancı pozitiflik sonuç oranının arttığını göstermektedir.
Abstract
OBJECTIVE: The aim of this study is to establish the place of blood flow changes of adnexal masses by Doppler ultrasonography in differential diagnosis of benign and malignant masses; to compare these results with ultrasonographic morphologic score system, tumor markers and histopathologic results. STUDY DESIGN: Sensitivity, specifity, positive and negative predictive values of thrombocyte count, Ca-125 value, ultrasonographic morphologic score system point, Doppler S/D, Rl and PI values of 59 patients who had diagnosis of pelvic masses and planned operation were calculated in order to determine their importance in the preoperative adnexal malignancy diagnosis. Positive and negative values obtained with diagnostic methods, were compared with histopathologic results of the masses. RESULTS: 41 of 59 cases had benign characteristics and remaining 18 had malignant characteristics. Ultrasonographic score system point (>5 malignant) and Ca-125 values (cut-off>35 lU/ml) of malignant masses were statistically significant. Doppler Rl (cut-off<0.6), S/D (cut-off<2.3) and PI (cut-off<1.0) values of malignant cases were found lower and statistically significant in relative to benign cases. CONCLUSIONS: PI value, one of the Doppler ultrasonographic measures, with a high sensitivity (%100) and negative predictive value (%100) seems to be the most useful parameter in evaluation of adnexal masses in reproductive ages for malignancy. We established that Ca-125 values, Doppler Rl, PI and S/D rates and ultrasonographic score system had similar specifity in differential diagnosis of malignant neoplasias. When all of the parameters were evaluated together, we established that sensitivity and negative predictive values (NPD) were high. So that the rate of false positive results increases, in spite of decrease in the rate of false negative results.
