Survival and prognostic factors of epithelial ovarian carcinomas: A multivariate analysis

Epitelyal over kanserlerinde, prognostik faktörleri ve onların sağkalım üzerindeki etkilerini araştırmak amacıyla 1990 ve 1997 yılları arasında öpere edilmiş olan 50 hasta değerlendirilmiştir. Olguların hepsine optimal cerrahi uygulanmış ve daha sonra cisplatin bazlı kemoterapi verilmiştir. Olgulara ait parafinize patoloji preparatları, histolojik tiplendirme, gradelendirme ve mitotik figürlerin sayılması amacıyla yeniden değerlendirilmiştir. Bağımsız prognostik faktörleri belirlemek amacıyla Cox's regresyon analizi kullanılmıştır. Multivaryant analiz sonucunda sadece mitotik indeks bağımsız bir prognostik faktör olarak saptanmış, dolayısıyla mitotik indeksin hesaplanmasının hastanın prognozunu belirlemede faydalı olabileceği sonucuna varılmıştır.
Abstract
OBJECTIVE: To identify prognostic factors and their effects on survival in epithelial ovarian carcinomas (EOC). STUDY DESIGN: Fifty patients who were operated between 1990 and 1997, with the diagnosis of primary EOC were enrolled in this study. They all underwent optimal debulking surgery, and then received cisplatin-based chemotherapy. Clinical records were reviewed to obtain information about patients' age, surgical stage, and clinical follow-up. Paraffin tissue blocks of the cases were reevaluated for histologic typing, grading, and counting the mitotic figures. The Cox's regression analysis was used to build a multivariate model for survival to identify independent prognostic factors and to assess relative risks. RESULTS: The mean age of the patients was 53 years (25-71). Median follow-up was 45 months, and 11 of the 50 cases died during follow-up period. The median survival rate was 75 months and overall survival rate was 72% after four years. In multivariate analysis, only the mitotic index revealed an independent prognostic factor (p<0.05). CONCLUSION: We propose that counting the mitotic index of the tumor is usefull for predicting a patient prognosis.
