Betanecholchloride: Is it effective to prevent urinary dysfunction after radical hysterectomy ?

Postoperatif mesane disfonksiyonunun tedavisinde kolinerjik ilaçların rolü açık değildir. Evre IB1 ve IIA servik kanseri tanısı ile tip III histerektomi uygulanan olgularda postoperatif detrussor hipotonisinin profilaksisinde betanekolün etkinliğini araştırmak için prospektif randomize bir çalışma planladık. Çalışma grubuna (15 olgu) günde üç kez 50 mg betanekol postoperatif üçüncü gün başlanarak rezidü idrar hacmi iki kez 50 ml olunca betanekol başlandı. Ortalama yaş, operasyonda çıkarılan vagen cuff uzunluğu ve parametrial uzunluk, suprapubik kateterizasyon süresi (10 gün ve 9.8 gün, p=0.8) ve rezidüel idrar hacminin <50 ml ölçüldüğü ilk gün (9.6 ve 9.62 gün, p=0.9) açısından gruplar arasında fark yoktu. Betanekolün radikal histerektomiye bağlı mesane disfonksiyonunu önlemek için profilaktik kullanımı etkin bir uygulama değildir.
Abstract
OBJECTIVE: We designed prospective randomised study on the effect of betanecholchloride for the prophylaxis of detrussor hypotonia after radical hysterectomy in cervical cancer patients, stage IB1 and IIA. STUDY DESIGN: The study group (15 patients) received 50 mg betanecholchloride three times a day from the third postoperative day until residual urine volume was less than 50 ml, measured twice. Control group (16 patients) received only betanecholchloride if the residual urine persisted above 50 ml after the tenth day postoperative day. RESULTS: There were no significant differences in average age, vaginal cuff size and parametrial size. Also there were no statistically significant differences in suprapubic catheterisation time (10 vs 9.8 days, p=0.8) and first day of residual urine volume less than 50 ml (9.6 vs 9.62, p=0.9). CONCLUSION: We concluded that a prophylactic application of betanecholchloride is not an effective to protect the postoperative bladder dysfunction due to radical hysterectomy.
