Özet
Tubanın primer koriokarsinomu oldukça nadirdir. Dünya literetüründe 60 kadar vaka rapor edilmiştir. Bu nedenle bir primer tubal koriokarsinom vakası sunuldu. 21 Yaşında G:1 P: 1 olan bir hasta beş haftalık amenore ve üç gündür süren alt karın ağrısı nedeniyle kliniğimize başvurdu. Abdominal muayenede sağ alt kadranda hassasiyet ve rebaunt pozitifdi. Bimanuel muayenede servikal hareket hassas ve uterus antevert idi.Transvajinal ultrasonografide douglasda mayi ve 5x6 cm ebadlarında miks eko gösteren adneksiyal bir kitle saptandı. ßhCG 78000 idi. Hastaya laparotomi ve sağ salpingoooferektomi yapıldı. Histopatolojik sonuç primer tubal koriokarsinom olarak rapor edildi. Cerrahi sonrası metotreksat, aktinomisin ve siklofosfamidden oluşan üçlü kemoterapi verildi. Hastamızda 16 aydır koriokarsinomla ilgili bir bulguya rastlanmadı. Ektopik gebeliğin tanısında ve takibinde ßhCG monitorizazyonu önemlidir. Özellikle yüksek ßhCG seviyeleri nadir ancak öldürücü olan gestasyonel koriokarsinomun bir belirtisi olabilir
Abstract
Primary choriocarcinoma of the fallopian tube is extremely rare. Nearly 60 cases have been reported in the world literatüre. For this reason, a primary tubal choriocarcinoma is presented. A 21-year-old woman, gravida 2, para 1 presented to the our clinic complaining of lower abdominal pain for three days and 5 weeks amenorrhea. Right lower quadrant tenderness with rebaund was positive on abdominal examination. She had cervical-motion tenderness and an anterior uterus on bimanuel pelvic examination. Transvaginal sonogram showed fluid in the cul-de-sac and right adnexal mass (5x6 cm) with mixed echo pattern. Her quantitative serum ßhCG was 78.000 mIU/ml. The patient underwent exploratory laparotomy. We performed right salpingo-oophorectomy. At the pathological investigation primary choriocarcinoma of the fallopian tube was revealed. After surgery combined chemotherapy consisting of metotraxate, actınomycine and cyclophosph a mide(MAC) was given. The patient has remained free of disease for 16 months. Monitoring of ßhCG levels is important in the management of ectopic pregnancy. Especially high ßhCG levels may be a sign of gestational choriocarcino ma, a rare but mortal condition.
