Deep venous thrombosis in Turkish pregnant women

Özet

Bu çalışmada hamile Türk kadınlarındaki derin ven trombozu insidansı, hastalık özellikleri ve risk faktörlerini araştırmak amaçlandı. Aralık 2000 ve Mart 2002 tarihleri arasında tromboembolitik hastalık tanısı konulan, 26 gebe hastanın dosyaları incelendi. Bu süre içinde derin ven trombozu insidansı 1000 gebelikte 0.91 olarak hesaplandı. Derin ven trombozunun en sık görüldüğü trimester % 46 ile ikinci trimester olurken, bunu %42 ile üçüncü trimester izledi. Derin ven trombozu %70 oranında sol ekstiremitede izlenirken %26 oranında sağ ekstiremitede ve % 3.8 olarak da bilateral izlendi. Vakaların %76'sında derin ven trombozu baldır damarlarında idi. Yapılan çalışmada derin ven trombozu insidansı literatürdekilerle benzer oranlarda izlendi. Sonuçta derin ven trombozu sıklıkla son iki trimesterde, özellikle sol alt ekstirimitede görülmektedir. Gebeliğin ikinci trimesterinden sonra görülen izole sol bacak şişliği ve ağrısı derin ven trombozu riskini akla getirmelidir.

Abstract

OBJECTIVE: To evaluate the incidence, disease pattern and risk factors for deep venous thrombosis (DVT) in Turkish pregnant women. STUDY DESIGN: A retrospective analysis of pregnant women having DVT, from December 2000 to March 2002 was conducted. The incidence and risk factors for thromboembolitic disease were evaluated. RESULTS: 26 women were diagnosed as having DVT during the study period. The total number of deliveries over the study period was 28568, giving an incidence of 0.91 per 1000 deliveries. The event rate for DVT was 12% during the first trimester, 46% for the second trimester and 42% for the third trimester. 26% of the cases were right sided whereas 70% of the cases were left sided. 3.8% of cases were bilateral. In the majority of cases, DVT was limited to calf veins (20 cases, 76%). CONCLUSION: The incidence of thromboembolitic disease is in Turkish pregnant women are similar to that in European population. In conclusion it was noted that DVT has a higher incidence in antepartum period, especially in the last two trimesters, this may be explained by the events that impact on coagulation systems of pregnancy. DVT is mainly seen in left leg. Isolated leg swelling and pain, especially in the left leg or inguinal region should alert the obstetricians and midwives about the possibility of deep vein thrombosis.
