Cytological effects of transdermal and oral estrogen therapy on vaginal mucosa

Özet

Oral konjüge equine östrogen veya transdermal estradiol tedavisinin ürogenital system üzerindeki etkilerini araştırmak amacıyla postmenopozal dönemdeki 350 kadın, hormon replasman tedavisi öncesinde ve bir yıllık tedevi sonrasında degerlendirildi. 28 hasta oral yoldan 0.625 mg. konjuge östrogen alırken, 222 hasta transdermal yoldan 50 mikrogram estradiol aldı. Her iki tedavi protokolu bir yıl boyunca surekli olarak uygulandı ve uterus varlığında tedaviye her iki grupta 2.5 mg. medroksiprogesteron asetat eklendi. Oral grupta 31 hastaya ve transdermal grupta 83 hastaya histerektomi yapılmıştı ve bunlar sadece estrogen tedavisi almışlardı. Tedavinin etkinliği anket formları (lokal şikayetler için urogenital index), jinekolojik muayene, vaginal sitoloji (Maturasyon index), vaginal PH, Döderlein basil oranları esas alınarak belirlendi. Tedavi öncesinde ve sonrasında hastalar urogenital şikayetler, vaginal sitoloji, vaginal PH ve Döderlein basil yüzdesi açısından karşılaştırıldığında gruplar arasındaki farkın istatistiksel olarak anlamlı olduğu ortaya kondu p<0.00l). Sonuc olarak her iki tedavi modelinin ürogenital şikayetler ve vaginal fizyoloji üzerindeki etkilerinin benzer oldugu saptandı.

Abstract

OBJECTIVE: To investigate the effects of conjugated equine estrogens and transdermal estradiol treatment on urogenital system. STUDY DESIGN: 350 postmenopausal women were evaluated before estrogen replacement therapy and after one year of treatment. 128 patients received oral conjugated estrogen 0,625 mg. and 222 patients were treated with transdermal estradiol- 50 micrograms. Both of the therapies were administered continuously and associated with medroxyprogesterone acetate 2,5 mg.daily in case of an intact uterus. 31 patients in the oral group and 83 patients in the transdermal group had been hysterectomized and they received only estrogen orally or transdermally. The effectiveness of the therapies were evaluated on the basis of questionnaires (urogenital Index for local complaints) and gynaecological examinations which included assessments of vaginal cytology (Maturation index), vaginal PH and the percentage of Doderlein bacillus. RESULTS: There was a statistical significance between pretreatment and posttreatment groups for urogenital complaints, vaginal cytology, vaginal PH and percentage of Doderlein bacillus (p<0.00l). CONCLUSION: It was concluded that both of the estrogen administration routes has similar effects on vaginal physiology and urogenital complaints.
