Özet
Bu çalışma Sivas bölgesindeki vajinal mayaların tanımlanmasını ve direnç patternlerinin saptanmasını amaçlamaktadır. Biz 6 haftalık bir süre içinde Doğumevi Hastanesine başvuran 623 kadından vajinal örnekler aldık. Vajinal sürüntüler sabaraud dextrose agara inoküle edildi. Üretilen tüm mayalar candifast kitleri ile (International Microbio/Stago Group, France) tanımlandı. Direnç tanımlanması da yine aynı kitler kullanılarak yapıldı. Kültürlerde 61 C. albicans, 10 C. Glabrata, 7 C.kefyr (pseudotropicalis), 6 C. albicans var. stelloidea, 6 C. tropicalis, 5 C. parapsilosis, 5 C. krusei, 3 Saccharomyces boulardii, 1 C.guillermondii, 1 Saccharomyces cerevisiae, 1 Trichosporon olmak üzere toplam 106 maya mantarı izole edildi. Identifikasyon candifast kitleri ile karbohidrat fermentasyon özelliklerine göre gerçekleştirildi. Kitlerde kullanılan eşik değerler uyarınca 51% fluconazole, 42% myconozole, 31% ketokanozole, 53% euconozole, 3.7% amphotericin B, 1.88% nystatin and %1.88 flucytosine in vitro direnci gösterildi. Bu sonuçlar birincil sağlık kurumlarında antifungallere direncin arttığını göstermektedir. Mukozal maya infeksiyonlarının önemi tedavide klinik birikimlerden mikrobiyolojik işlemlere kadar değişen bir yelpazede çoklu disipliner yaklaşımların gerekliliğini vurgulamaktadır.
Abstract
OBJECTIVE: This study aims to identificate vaginal yeasts and determine the resistance patterns of the microorganisms in question in Sivas region. STUDY DESING: We obtained vaginal specimens from a total of 623 women applied to a state maternity hospital consequetively over 6-week period. Vaginal swabs were inoculated onto sabaraud dextrose agar. All the yeasts recovered were identified with candifast trays (International Microbio/Stago Group, France). The same kits were used to determine the resistance patterns of the yeasts RESULTS: The cultures yielded 106 yeasts including 61C. albicans, 10C. glabrata, 7C.kefyr (pseudotropicalis), 6C. albicans var. stelloidea, 6C. tropicalis, 5C. parapsilosis, 5C. krusei, 3 Saccharomyces boulardii, 1C.guillermondii, 1 Saccharomyces cerevisiae, 1 Trichosporon species. Identification is made by Candifast kits according to carbohydrate fermentation. We found 51% fluconazole, 42% myconozole, 31% ketokanozole, 53% euconozole, 3.7% amphotericin B, 1.88% nystatin and %1.88 flucytosine in vitro resistances according to breakpoints in the kits we used. CONCLUSIONS: These results indicate that resistance to antifungals are increasing in primary care settings. The importance of mucosal yeast infections emphasizes the necessity for interdisciplinary approaches for the management ranging from microbiologic procedures to clinical experiences.
